How do I recover vanishing playlists on iPhone 5s?

Have an iPhone 5s running the latest update. When I sync my phone to iTunes on my macbook, all the playlists disappear from my phone, but remain in iTunes. Not just the smart playlists, but all of them. When I restore my phone the playlists will show up again (after a long, tedious sync) only to disappear roughly 12-24 hours later. I have tried disabling iCloud, and i already have iTunes match turned off as it is garbage that is full of issues. All music synced remains on the phone, but playlists themselves delete themselves off my phone and won't come back, anyone having similar issues or know what to do?

E-mail is kept on your e-mail server.
Have you failed to import your photos to your comptuer regularly?
If so, then they are likely gone. You can try restoring from backup.

You cannot, unless they were there at last backup. You can try restoring from backup, losing anything done since the last sync.

Quick answer if you let iTunes manage your music: Copy the entire iTunes folder (and in doing so all its subfolders and files) intact to the other drive. Start iTunes with the option (shift on Windows) key held down and guide it to the new location of the library.

I would turn off the automatic syncing so that way you can see what is on your iPhone. From there, you should be able to see what is on your iPhone and then can export out the playlists and then import them back to your library. I would also recommend backing up your iPhone before going through with this process. To export a playlist, have it highlighted then go to File > Library > Export Playlist…

If your note is gone from iCloud.com there is only a few ways to possibly recover it.
The first things I would check is a iCloud Backup, a iTunes Backup, or a 3rd party email that may have contained the note. To restore from a iCloud or iTunes backup follow HT1766. This article explains how to restore a device from either back up.
Note if you have backed up since the note went missing, you will not have the note on the backup. Make sure you do not back up if trying to restore from a back up as this will back up the content without the note.
To get to the "Set up assistant" on an iOS device, you must go to Settings>General>Reset>Erase All Content and Settings. Once the phone restarts, you will be at the "Set up Assistant"
Gmail emails are very commmon sources for notes and you may have been adding them to the gmail notes without even knowing it.

You sync songs from your itunes library to each device.
All of your music should be in your itunes library on your computer. From there you can sync them to whatever idevice(s) you like.
