How do I remove Restriction Passcode if one is set up accidentally?

If I have restrictions set "On" under General Settings and can not remember my password, how do I remove the restrictions?  I tried restoring it but the Restrictions still show 'On'.

You have to restore the iPod to factory defaults/new iPod.

    How to reset?

The only way to remove a restrictions passcode is to save as much of your data as you can (import your photos and videos to your computer, back up your contacts to a supported program or by syncing them with a cloud service like iCloud, sync or email your notes to your computer, etc.), and then restore you phone as new, without using your backup.
