Your i-device was not designed for unique storage of your media. It is not a backup device and media transfer was planned with you maintaining a master copy of your media on a computer which is itself properly backed up against loss. To use a device with a new computer you transfer the content from the old computer (or its backup) directly to the new computer, not the device to the new computer. Syncing is one way, computer to device, updating the device content to the content on the computer, not updating or restoring content on a computer. The exception is iTunes Store purchases which can be transferred to a computer.

Yes, iCloud is taking place of Mobile Me over time. MM will be turned off in June of next year.
With iCloud, you can store the music in the cloud and download the tracks that you want to listen to without having to sync to your iTunes.

If I am understanding you correctly, it sounds like you are returning to an old iPhone that has been erased and you would like to set it up as a new device without restoring from a backup which would affect the name of your iPhone, as well as the contents therein. Basically, it seems like you may be stuck on a screen that is asking if you would like to restore your device from the backup that is filed under the name of your new iPhone, or if you would like to set up the device as a new device. Connect the old phone to your computer, right click on it in iTunes, and choose Transfer Purchases. That should put all of your music in iTunes except any tracks that were not purchased from iTunes.
