How do I unlock my Iphone 4 to use in Italy?
I am traveling to Italy soon, and I am "unlocking" my phone from ATT. Is there anything else I need to do, or can I just purchase a pre-paid sim card (for example from TIM) and use my phone as normal? Does my phone need to be jailbroken or anyother actions to make it work?
If AT&T is unlocking it for you, then no. Once unlocked it will accept sims for any carrier.
If you unlock it by any other means such as jailbreaking then you void your warranty, and will not get assistance from these forums or Apple.

Forgotten Restrictions Passcode Help
You will need to restore your device as New to remove a Restrictions passcode. Go through the normal process to restore your device, but when you see the options to restore as New or from a backup, be sure to choose New.

Could you not restore the device to an earlier version?
No, you cannot downgrade your iOS device.
That may mean jailbreaking, I'm not sure.
Jailbreaking is not supported by Apple or this forum.
Or what about replacing the motherboard?
There are no user serviceable parts on an iPhone. -

Turn the Phone Off...
Press and Hold the Sleep/Wake Button and the Home Button at the Same Time...
The Apple logo will Appear and then Disappear...
Usually takes about 10 - 15 Seconds...

If you are referring to a lock to a specific cell carrier, so you can use the iPhone with a different carrier, only the cell carrier to whom the phone is locked can authorize unlocking. You will need to contact the carrier.

If the carrier tells you that they do not unlock iPhones, then there is nothing you can do other than sell the iPhone to someone who can use it with that carrier and purchase an iPhone that is officially unlocked.

Unfortunately once the iPhone is in that mode the best practice is to just restore the whole iPhone via iTunes.
The iPhone is in recovery mode. So to get it out of recovery mode and restore through iTunes is all you got to do.
