How to create a form to be filled out on the iPad?

Hello,
I have created a daily report/timesheet pdf form to be filled out by our jobsite superintendents on their iPads.  I have tried everything (including downloading as many free-and some not so free-apps from the app store) and still cannot get my forms to work on the iPads.  Most of these will not even open my file.  I have no problem purchasing an app, but I would love to know that it will indeed work beforehand. 
My questions are:
Can I create a form with Acrobat Pro that can be filled in on an iPad? 
What iPad apps actually offer this function?
(If we make it this far) Are there any limitations to the complexity of the forms I create?  
Additional Info:  I emailed the pdf form document to myself as an attachment.  On my desktop, it opens like a normal PDF.  On my iPad it is labelled as "winmail.dat" and will only not open.

Does the PDF Expert app allow you to include drop downs, check boxes, etc in your forms?  At the moment, all I need the app for is to fill this form... naturally, we can find more uses for it as we use it, but I would hate to download it only to find that it won't work.  Could I by chance email you the form to see if it will work?   
Regarding the "winmail.dat" issue, I emailed it in HTML, Rich, and plain.  It shows up the same in every one.

    If the end user is using Adobe Reader to fill this out then no, it isn't possible.
    I can say whether it would be possible if all parties are using Adobe Acrobat but I doubt that it is.
    You chould probably look in to creating a web application that can generate a PDF.

    You don't want to use Preflight to flatten form fields since it is buggy for this. Instead use JavaScript (flattenPages method), or use this free JavaScript based tool: UVSAR : Selective Flattener tool for Adobe Acrobat 8, 9 and X
    Overprint flattening isn't relevant to this.

    I have created a form which will be filled out by customers and sent back to me via 'submit' button. What I am trying to figure out is when we receive the filled out form, is there any way to have it as a pdf. To clarify, when the form is filled out and sent, the receiver opens the email and attached is a non-editable pdf. This way the customers information is no longer editable.

    I'm a DJ and wedding season is coming up.  For each wedding that I do, I usually give the bride and groom a list of my songs and they choose a bunch of songs that they'd like to have at their reception.  In the past, I've just been sending them a webpage word document with the songs and they'd send me their selections in a different document or directly in the reply email.  I'd like to make this process easy for them (and myself) by making a pdf document where they can simply place a checkmark beside the songs that they want and then send it back to me.  As a bonus (if it's possible) i'd like to be able to export the songs that they selected into a separate document so that I don't have to go through the entire pdf (with thousands and thousands of songs) when they send it back to me.
    I would be ever greatful if somebody help me or direct me to a link with some additional information.

    You would need to first create the form ( or Widget in EchoSign) first to generate a url which you can paste it in the email being sent to customers. You would need to make sure you have a Document Cloud Enterprise Premium account in order to create a Widget. If you don't have the account, you can use the trial version to test it.     You cannot create a Form from code at run time. 
    For one thing, the user running your application will, almost certainly, not have  a copy of Visual Studio to hand with which to build anything.  (Not that you actually need 'Studio at all; Notepad and csc.exe are all you actually
    need), but that's another story).
    You can do this with an awful lot of pain, using the innards of the compiler from iside your own program, compiling the new Form's source code on the fly and then executing the resulting assembly within your application.  IMHO, that's a simply terrifying
    prospect especially as the code you will be compiling will be on someone else's machine and over which you will have absolutely no control.  If you're lucky, they'll spell something wrong and it won't compile.
    The "usual" way to do this sort of thing is to create additional, "plug-in" forms as library assemblies and have your program load and execute these at run-time.

    Short answer: no. You'd have to use some other software (Dreamweaver for example) to setup the form and the form management script.
    Contribute is basically only good for one thing: editing existing pages within a Dreamweaver template environment.

    I am needing to create a form that will display database information and also go out to a windows shared location and read a JPG image back into the form. I creating a Rolladex of sorts.
    My question is I am total 100% new to forms I have never created one or been shown how to create one can anyone recommed some place on the interenet I could get a how to maybe a sample that I could create and test so that I can begin the learning process.

    Be aware that most of the documentation for Forms 9.04 and 9.0.2 apply to 10.1.2.0.2 in most cases. This may be helpful to know since there are more docs for these older versions.
    That said, one of the strengths of the Forms Builder is its Wizards. Without writing any (or at least very little) code you can create a basic application. If you understand the Oracle Database and have a basic knowledge in PL/SQL you should have no problem. Of course a working knowledge of how a web server works is also help since you will need Oracle Application Server to deploy your app if you ever go into production with it.
