HOW TO DESIGN COBRA FORM

Hello Experts,
As I'm new to HR-ABAP--COBRA Forms ,
Can anybody tell me to design a COBRA Form for Info type 0167(Certificate of Group Health Coverage )?
When Searching I got some functional settings through SPRO.But here my requirement is to Design the COBRA FORM for Group Helath Coverage.

I am not sure how you can have two different languages inside the same PDF.
Alternatively, if you want to place only the static labels, you can convert all the static label in one language to images and set the form language setting to other. And place the images inside the form using Image field. For example, you can keep the form language as English and then convert all the arabic language labels/ Static text to images.
Anotherway,
Have same sections in both the languages and hide and unhide based on the user selection of the language..
if your user chooses Arabic, then you can unhide the Arabic sections and change the form language properties programmatically..

How to design a form to calculate values for various fields

I have designed a form (in Adobe Live Cycle ES) which has fields for height (in inches), weight (in pounds) and serum creatinine (a decimal field). I have created a field for weight (in kilogram), a field for Body surface area (in m2) and creatinine clearance (in ml/min).
The weight can be derived by dividing weight in pounds by 2.2
The body surface area is calculated by
BSA (m²) = ( [Height(in) x Weight(lbs) ]/ 3131 )½
The creatinine clearance for males is calculated by
(140-age) * (Wt in kg) / (72 * Cr) for males
Creatinine clearance for females
(140-age) * (Wt in kg) *0.85 / (72 * Cr).
At another field I want the form to calculate the dosage by simple multipication of BSA with the dose basis or weight with dose basis.
How can I build an action which will perform these functions and place the value in the appropriate field?
Can some one help me and tell me the exact steps.
I would send the form I designed, if asked.
PS: I do not know java language.

Help in design a form

Hi all
I have to write a simple student application for w/r/manage mail based on a web form.
I don't have a clear ideas regarding a main form: it must visualize all the incoming mail with the fileds "from", "subject" and "data"; this mails are stored in a local database after download.
How could design this form? I mean, I could generate a table with this fields, and every time I'm downloading new mail I remake all the form or there is another way for append single lines?

With a web application most often what happens is that the entire page gets remade every time (take a look at hotmail)
You can use Ajax to just pass data back and forth, but that would be beyond a "simple" application.

How to use more than one page to design a form-like report???

It's not my interest to explain to you how to do something but I do feel obligated to tell others that what you're telling them is not completely safe. Yep, that's basically what I told you over a year ago. I know that some have been using your email script to inject mail through the headers for sometime now. No one is sending you emails through your contact form anyway since the spry validation is broken, so injection of your server script is the only way to send emails from your contact page for users who have javascript enabled anyway. Perhaps you will now realize that your suggestions may be hurting others rather than helping them, opposite of what my posts regularly achieve. Discoverer viewer dont support images or logos....Images are only supported in discoverer plus.

How do I design a form using the "Add" button?

I am trying to design a form where I can just insert a "Add Shipments" button and the form will replicate to the amount of shipments indicated. Lets say, I have six shipments...I can add a button but nothing happens.

The main thing we are trying to do now is to make a sub drop down menu (i.e. parent-child drop down menus). This means in some sections, like the ‘Orders Received’, when they select how many shipments they received, a corresponding group of further RFIs pop up for each shipment. Right now I just have a couple of these sections under the tab where they select how many shipments. Being able to do this will make the form shorter and easier to read.

Still no attachment ....you have to attach while connected to the forum. You cannot do it via the email.

Multi-record / spread table...moveable fields in designer generated form

Good morning all;
I hope you are doing good...
Imagine a multi-record block layout with it's overflow property set to spread-table to accomodate 10 varchar2(25) fields. Now, of the hundreds of users working with this generated form, not one of them wants to see the fields in the same order on the screen.
What i would like to do is generate the form from designer so that a user could 'drag'n drop' let say, field #10 to the field #2 spot on the screen. Something like windows explorer where you can move the 'Size' column around so it is viewable without having to use the spread-table bar at the bottom of the screen...
Any idea on how to generate this form from designer?

I don't know a solution, but I think a solution could be easier found if it was a read-only block. You did not specify that. Then maybe you could dynamically change the query behind the block.

Calling a report from designer genareted form

i will appraciate if anyone can tell me how to create a form module that navigate to a report module in 9i designer.i tried action item but i receieved couple of different error messages.i have also tried to find information on this but i couldnt

Okay, this is gonna be tricky to explain but lemme give it a shot:
I'm assuming that your Report is a Called Module inside your Form. This allowed you to create an Action Item that calls this Report upon pressing the provided button. Now, generate your Form and open the .FMB. Try to locate these two things:
1. The Report attached to your form, with the Designer-provided name "CG$OFG_REPORT", and
2. A Program Unit named "CGNV$AI_(name of action item)" (e.g., I named my action item "FAX" so the prog. unit is named "CGNV$AI_FAX").
If you dig into the Program Unit's code, you will find that it makes use of a procedure named "CGNV$INVOKE_REPORT" which resides in the standard, provided PL/SQL Library "OFGNAVL.PLL" . I looked for my Ofgnavl file and opened it up to inspect the CGNV$INVOKE_REPORT procedure. I found that it has been hard-coded to call the report "CGNV$OFG_REPORT", and not "CG$OFG_REPORT" !!! That's probably why your action item is failing.
Inspect the procedure a little further. There are about seven lines, one after another, that all invoke the SET_REPORT_OBJECT_PROPERTY built-in procedure, and then the RUN_REPORT_OBJECT immediately after. What I did was to copy this code, go all the way back to Designer (Design Editor), and build myself a procedure under my module's Application Logic, Named Routines which calls the correct report (I even varied this slightly by specifying the report by it's filename). I even added some code to put out some feedback (i.e., Message) to tell the user when the report has finished successfully.
To make a long story short, I wrote a procedure to invoke my report, and then I just created a button that calls the procedure when a user presses it.

I think the confusion is that you are not being clear enough with which side of this process you need help with.
1) Do you need help with sending the request from the phone to the server?
or
2) Do you need help parsing the received data on the server side?
If 1 then you can use a NSURLRequest to submit the data. It will need to be encoded as a multipart form since you are attaching a file.
If 2 then this isn't the correct forum for helping you write a server side application.

How to send a form in DW 7?

Hi! I'm from Spain and I'd like to be understanded if possible. I've got Dreamweaver MX 7. I'd like to know how to send directly the form:
I design a form, I put a text box with "name", "e-mail" and "comment" for example and the button "send". If I test it, when I press send, the Outlook (I've got windows XP) is opened with the information.written in the form.
Can I send the form without opening the Outlook?
It is possible to do it with HTML language?
Where do I have to write the script?
If it isn't possible in HTML, how do I do it in PHP?

these extensions make it incredibly easy to do this very task. determine first how your server email is being handled... through phps built in mail function, or does it require smtp authentication to relay mail.

How to call ISR form in task

Am working in ISR workflow for address change. I have designed ISR form in SFP t-code. I have designed the workflow for my requirement. How to call the ISR form in my method. I have check other standard workflow in that its using EDITASYCNOROU method with BUS7051. I have CHECKED the code in that it calls transaction iqs22. Since am executing from portal. I cannot use call transaction method. I have even checked in SWFVISU t-code there is no standard task for ISR. ALL the category is available for javawebdynpro,ABAP,BSP. What is the category that ISR falls.
