Not gonna happen. Return the phone for a refund, otherwise you won't be able to use it. More than likely, this phone was stolen to begin with.

    Are you the first owner of this phone, or did you buy a previously used phone?
    Is the Apple ID that the phone is locked to your ID, or somebody else's ID?

    From where did you purchase the iPhone? Are you certain it really was factory unlocked? I can't say that an iOS update couldn't inadvertently lock an iPhone, but more commonly it would happen with a jailbroken iPhone. If by "from Irvine" you mean Irvine, California, then you don't have a factory-unlocked iPhone unless you purchased it from another country, as there are no unlocked iPhones sold through any authorized reseller in the US.

    Not covered by warranty. If you make an appointment at an Apple store, you will be offered an out of warranty replacement. Cost is US $199 for an iPhone 4S.

    If you bought it directly from Apple as unlocked it should be unlocked for all networks. There are phones sold that WILL lock to the first carrier it is used with, but this does not apply to phones where the receipt says "unlocked".
    I have not had good experiences with Carphone Warehouse.

    AT&T can only unlock a phone that is locked to them. There is no indication that Sukhbir had an iPhone that was locked to AT&T, though it was possible.
    And no, if the iPhone is officially unlocked by the carrier, there will be no problem from Apple or anyone else for using that iPhone with any other carrier, nor will the phone relock if restored or upgraded. The only problems come if the user jailbreaks the iPhone to unlock it.

    The online Apple Store has a limit of two iPhone 4S units per purchaser. I would suspect that the physical Apple Stores have the same limit. But as Tim said, you need to call the Apple Store from which you would like to purchase and ask, though again, they almost certainly will not have any in stock.

    Apple do not lock iPhones
    Carriers do because they have subsidised the purchase price
    and therefore only the carrier can unlock an iPhone
    Orange have decided to apply 6 months to protect their revenue
    they never have been the fastest to unlock

    To activate your phone, you need a SIM card from the carrier to whom it's locked.  The SIM needn't be active, but it needs to be present, nonetheless.

    RajaRani is correct. The only CDMA carrire you can use a Verizon iPhone 5 on is Verizon. You can use it on whater GSM carrier you like. Note that you will most likely only get 3G speeds.
