HP Recovery usb device failed

have a new HP laptop:
HP ENVY 15t-j100 Select Edition CTO Notebook PC (ENERGY STAR)
 Product Number: E9W31AV
Several hours after its first boot the laptop failed to load and I got a message saying it can't load win 8.1 os.
(this was two month ago and since then I am trying to recover it without any success).
HP US support sent me a recovery usb device with win 8.1
Before connecting it and starting the recovery:
1. I did a System Diagnostics. All Passed (memory & hard drive).
2. I reset the bios to default factory settings (and change the 'usb 3.0 configuration in pre-os to Auto, in case the usb is 2.0).
3. I connected the recovery usb directly (no hub).
I started the recovery...
I finished the "factory image recovery preparations" stage successfully:
I entered Skip and after that i removed the recovery usb device and continue the recovery by pressing "Continue":
After that the laptop boot and started "Software Installation" stage:
After several minutes it boot again and I got the following screen:
I saved the recovery logs to an external device and this it is saying:

I did everything "by the book", got the same errors 5 times for the above scenario and 5 other times for other bios config (enable\disable features).
What is the problem?
It is Hardware issue?
configuration issue?
Something related to UEFI \ RAID ?
Is it possibile to solve it myself?
Is there a tool i can use?
I am living outside the US and I don't have an option to send the laptop to HP lab in the US, but Only in my country - Israel.

    This is NOT resolved.  The "Upper..."Key in the registry editor doesn't exist as you can see below in the image I am providing.  This is still an OUTSTANDING issue, can someone please help with resolving this issue, super frustrating.
    This is not just an iPAD issue, I get the same problem with my iPhone as well, both (iPhone and iPad) have the latest iOS 6.1.3 installed as well.

  • USB device failed, when I plug in iPhone 4S running windows 7 (64bit)

    When I plug in my 4s it says USB device failed. In details it says that windows terminated the process (error 43) I tried telling it to use the apple mobile device driver, but that errors as well. I last synced this phone in June, with no issues. I have uninstalled and re installed iTunes. I'm scratching my head now. Any help is appreciated

    I have iphone 3g. I use Win 7 pro OS. when i plug in iphone to usb port, i get error message as - device driver failed to install - in detail - mtp usb device failed..
    In iTUNES i am able to view the iphone, sync my contacts with MS outlook etc..

    What a load of hogwash.
    I've just had this error come up - WITH NO CHANGE OTHER THAN UPGRADE OF IPHONE 4 TO IOS 5.1.1.
    (running iTunes 10.6.1.7 on 64 Win7)
    Powered phone off, and back on again - same error... Haven't had to reinstall windows in a few years - maybe iTunes is now the least stable software in my collection - Might have to try the two (button) salute to Apple...

    When connected N97 via cable to pc ( windows 7 )  " MTP usb device failed" and cant see N97 in Pc suite. In mass storage mode work fine.
    Please HELP.

    Download ovi suite for n97 and your issue will be resolved.
    When u connect ur n97 for the first time after installation make sure to press install on whatever the phone is being prompted to install. Oh and one more thing .. make sure you restart your phone and your pc before doing all this

  • USB devices fail to mount

    After installing 10.4.6 my USB devices fail to mount. No icons on desktop.
    If I go to Apple menu/ Apple system Profiler/ more info/ USB, they show up OK with all correct information.
    If I go to applications/ utilities/ disk utility, they do not show up so I can mount them. In fact, disk utility spins forever. Removing the USB devices, disk utlity works OK.
    Another post suggested to reset-nvram, then reset-all. Things happened, but it did not help.
    These worked fine before the update.
    I have tried each device through a hub and directly to the computer. Still no good.
    The devices are a Dazzle compact flash reader and a Nikon camera.
    Any help would be appriciated.

    Have you tried plugging in the devices one at a time to see if the problem is with just one of them?
    If so, you may need to contact the manufacturer & see if they say it is compatible with 10.4.6 as is, or may need a firmware update, which hopefully they can provide.
    Note: when I Googled "Dazzle compact flash reader OS X" I got several hits suggesting at least one model with this description isn't very compatible with OS X, like this one.

  • USB Device Failed Detection

    Hello,
    I am trying to connect a USB to SATA/IDE Adapter to a USB port on Solaris 8. In the /var/adm/messages file, two messages show the following:
    usba: /pci@1f,0/pci@1,1/usb@c,3 (ohci0): getting device status failed (-1)
    usba: /pci@1f,0/pci@1,1/usb@c,3 (ohci0): connecting device on port 1 failed
    It appears that the USB device is being detected but the OS doesn't know how to define it. Maybe a device driver might be needed?

    I am trying to connect a USB to SATA/IDE Adapter to a
    USB port on Solaris 8. IIRC Solaris 8 only supports USB Keyboard and mouse. You'll probably need to get a newer version of Solaris.

  • NFS export external USB device fails

    I am trying to NFS export a FAT32 formatted external USB device, which fails with the error:
    /sbin/nfsd: Can't export /Volumes/<external>: Operation not supported (45)
    I am able to export internal/HFS drives, which have the "Owners Enabled: Yes" attribute, and therefore assume I need to set the flag accordingly on my external drive.
    Despite the fact that the device has been assigned a uuid (it appears to be in place in .fseventsd and running 'repair disk' echos it in syslog), I get this error when running vsdbutil:
    vsdbutil: Couldn't update volume information for '/Volumes/<external>/': Invalid argument
    vsdbutil: no valid volume UUID found on '/Volumes/<external>/': Invalid argument
    And diskutil returns this:
    Permissions are not enabled on the disk (-9973)
    I attempted to add the uuid to /var/db/volinfo.database in order to set the permissions there, to no effect.
    I don't believe that I am the only person who has attempted this, but I can find no evidence to the contrary. Thank you.

    NFS exporting requires specific NFS serving support from the file system.
    Unfortunately, the "msdos" file system implementation doesn't currently
    support NFS exporting.
    If you'd like that support added, I would strongly encourage filing a
    bug report/enhancement request with Apple:

  • Various USB devices failing

    I've just received a Hexacore MacPro (2013 style), but am having problems with some USB devices.
    I can see a lot of chatter on this, but not relating to my specific problems.
    The two devices I have which are consistently failing, are a Virus Ti and a Steinberg eLicenser dongle for Cubase.
    I've tried various combinations of connecting these devices both directly and via a Belkin 4 port USB2 hub (supposedly MTT enabled).
    The cubase dongle throws various different errors, but all relating to no connection. Sometimes it works for a period, but then fails again.
    The Virus mostly doesn't work, but when it does, there's a lot of digital noise.
    Both devices work perfectly on my 2011 MacBook Pro, which has native USB2 ports.
    Other USB devices seem to be working OK.
    It seems that either there's a failing in Apple's USB drivers for USB3 on MacPro, or maybe Apple has started using a new USB chip?
    Does anyone have any thoughts on this one? I bought this specifically for music production, but am hitting a dead-end.

    Hi. Yes, I have support tickets open with both.
    I'm seeing various web pages which mention hacking the system to force old USB drivers as a fix. I'm not on my own with this issue.
    By the way, both those devices are USB1 (12mb).
    That sounds somewhat drastic though.

  • KDE mount USB device fails

    Hi,
    I am running KDE and having problems mounting USB devices via the KDE Device Notifier in the task bar. When I insert the USB stick there is the normal little popup giving me the option to open the flash drive in dolphin. When I click it, I get the following error:
    "Could not mount the following device"
    Mounting the device via terminal works fine. I also put my user into the "storage" group like the wiki tells me. Still no change.
    Any ideas?

    You are my personal hero for today, I was suffering the same problem. Manifests itself as 2 CK sessions listed by ck-launch-session, with the wrong session being marked as active.

    Hi, ray008. In addition to what farby said, you can also check this link: http://www.microsoft.com/en-us/mobile/support/faq/?action=singleTopic&topic=FA141649. The link will work regardless of your location. Make sure that you are using the USB cable that came with the phone. Also, try connecting it to a different port. Keep us posted.

    You may have to reinstall the drivers, 
    Make sure you have an active data connection
    Plug in the phone
    Hit WINDOWS+PAUSE/BREAK
    Select device manager (top left)
    Find the device, it will probably have a yellow triangle with exclamation or question mark
    Right click the device and select uninstall, opt to remove files if asked
    Unplug the phone and re-attach
    make _sure_ driver install completes and do not remove again unless you are certain it has
    Should be fine now

    I want to access to the Internal Storage of my iPad 2 on Windows 7.
    But I have an error with the MTP USB Device => This device cannot start. (Code 10)
    So I cannot see my iPad in Windows Explorer.
    I tried many actions from Forums :
    update the driver to the "Apple Mobile Device USB Driver" located in "C:\Program Files\Common Files\Apple\Mobile Device Support\Drivers" 
    delete the upper and lower filters in REGEDIT : {36FC9E60-C465-11CF-8056-444553540000}.
    stop and restart the service "Apple Mobile Device"
    But as soon as I connect again the iPad there is the same error message after Installing device driver software => "Device driver software was not successfully installed" : MTP USB Device : Fail
    Is there someone who get the solution ?

    I have a similar problem, only mine is Code 31 for the MTP USB Driver.  The Apple driver loads fine, and I'm able to sync with iTunes, but I can't connect to the device as a drive and copy the photos off of it.
    I've tried nearly everything at this point that has been suggested in other threads, and nothing has seemed to work.

    Hello lwalbring and welcome to the Palm forums.
    Your Visor, as you suspect, does not have Bluetooth, so you must use the IR HotSync option.  Since you are using Windows 7 64-bit, I believe that you are going to have to upgrade to Palm Desktop 6.2.2 to make things work from an OS/Driver perspective.
    Since you are using such an old device, you are also going to have to download and install the PalmHotSyncSetup Utility from Pimlico.  This update turns on support for old Palm OS PDAs in Palm Desktop 6.2.2.  Without the update, you won't be able to sync your Visor with Palm Desktop 6.2.2.  The software is free and the link is all the way at the bottom of the screen.
    Lastly, if your PC doesn't have an IR port on it, you will need to purchase a USB to IR adapter.  Some laptops still have IR ports and most desktops don't.
    Once you have all the pieces, you'll want to go back to the Windows 7 and Vista HotSync thread again, and follow the directions for setting up and configuring IR HotSyncs.

  • N97 MTP USB Device

    Before I upgraded my N97 Firmware to 21.01.045 My pc windows 7 saw my phone as N97 when i would open a window to explore the phone contents, and when I would copy a video onto my phone it would convert it when it's copying it onto my phone automatically.
    After the update-I had problems when ever I connected my phone to the pc USB, telling me the MTP USB Device failed.
    But then all of a sudden few days later the MTP USB was successful but it again saw my phone as N96, BUT NOW it does NOT copy the videos as it used to, I have to go through Windows Media Player to transfer videos to my phone.
    Inconvinent...

    Solved by repairing installation of Nokia Suite. I'm using version 3.3 of nokia suite on a windows 7 ultimate operating system. Needed this step to update firmware.After many attempts I finally overcame this problem by going to control panel>uninstall a program>scroll to nokia suite>select uninstall/change>dialog asks if you want to repair or uninstall>select repair.
    *Please make sure of the fllg:
    >phone must be on
    >MUST be connected in PC Suite mode - If your phone tries to connect to internet or wlan, keep hitting cancel till it asks you to select (PC Suite) mode.
    >Windows may try to install drivers and you'll see MTP usb device installation Failed - ignore for now
    >Make sure the internet connection on your PC is active
    >NOW do the repair-install as described above
    >During repair windows will again install your device drivers, and this time you should be good to go
    Hope this helps someone, and saves them the many hours I took to get this right.
