how do you restore your ipod touch 4g when it says ipod is disabled connect to itunes?

The iPod should appear in the left sidebar of iTunes under Devices. You will have to plave the iPod in recovery mode for it to appear.
try using the steps in this article to force the device into recovery mode.
If you  have iTunes 11 turn on the Sidebar. Go to iTunes>View and click on Show Sidebar. You can also do a Crtl+S to show the sidebar.

    You have to restore the device within iTunes. You want to use the same computer that you always sync with so that you can restore your app data and settings. You can restore with any other computer, but you will lose everything on the iPod.
    Take special note of the article about using recovery mode. Many times, that is the only way to bypass the pass code and restore the device.

    - Try on another computer
