How do I deauthorize an old Iphone?

You need to have itunes on your PC.
Go to your account on itunes and look for Manage Devices under the heading of (iTunes in the Cloud). You should be able to see it more or less straight away once your in your account.
Simply click into manage devices and remove any you wish to be rid off.

    You cannot yourself deauthorize a computer that's not working or to which you no longer have access. You can authorize a total of up to five systems, so unless you've hit that limit it's probably not really important that you deauthorize your dead unit now. Once you reach five total authorizations, a button will appear in your iTunes Store account that will allow you to deauthorize all your systems, so that would at that time recover the now-useless authorization.
    If for some reason you really feel you need to deauthorize a system and you're not at the five-system limit, you can try contacting  the iTunes Store customer support department through their Contact form (follow the on-screen instructions)  and explaining the problem to them. They might be willing to clear your authorizations for you.

    iPhones (and iPads and iPods) aren't authorised or deauthorised, only computers are. If you are giving the phone to somebody then log out of your iTunes account on it by tapping on your id in Settings > Store, and then do Settings > General > Reset > Erase All Content And Settings to remove all of your content from it and put it back to factory defaults.

    Individual computers can only be deauthorised directly on them, but if you've reached the 5 computer limit then you can log into your account and 'deauthorise all' (which you can do once every 12 months) and you can then (re-)authorise the computers that you still have and need : authorising and deauthorising.

    If you do not have the PC anymore and want to unauthorize it you can unauthorize all PC's by going to the iTunes Store and signing in. Once signed in click the Account button which is the part where you Apple ID appears once signed in. You will be asked to enter your password again. Once you do so, click view account. In the Account Information Window,  beside 'Computer Authorizations' click Deauthorize All.     You will need to restore your phone and when prompted use the backup that you made yesterday.
    Before you do so, you may wish to change your wife's iCloud login details, otherwise you may find that you then overwrite her data. She should have her own iCloud account to avoid this.
