I have downloaded the latest iOS and it asks for a pass code which either I cannot recall, two failed attempts,. How do I get one so I can install

The iOS upgrade is seeking whatever code you use to unlock your iOS (iPhone or iPad) device; your device passcode or PIN or password you're using to secure your device.
If you've forgotten the PIN or password that you had originally configured to unlock your iOS device, there is a reset sequence.

I have just bout a brand new ipad mini and commenced setting it up. I had to leave it for a while and switched it off. When I came back to switch it on it asked for a pass code and I haven't set one up as far as I know. Anyone know whats going on!

Do a clean reinstall and delete the Firefox program folder before reinstalling a fresh copy of Firefox.

The Passcode is the 4 digit code you use to unlock your iPad.

Updated my iPad to IOS7. Now is asks for a pass code, but I've never used a pass code on it before.
