i never set up security questions and i dont know how to change them

I´m just guessing here but, your being told something like "Apple security enhancements" and that it tells you to create/verify your security questions when you try to purchase an application/music/etc from your
iPhone 4S, after updating to iOS 6?
If so
- Double tap you home button and close all application that´s running on you iOS device
- Go to Settings > Safari > Clear History and Clear Cookies and Data
- Open App store, download a free application and enter/verify your information

I got some help from an apple assistant on the phone. It is kind of round about way to get in.
Here is what he said to do and it is working for me...
a. on the device that is asking you for the security questions go to "settings", > "store" >
      tap the Apple ID and choose view"Apple ID" and sign in.
b. Tap on payment information and add a credit/debit card of your preference then select
      "done", in the upper right corner
c. sign out and back into iTunes on the device by going to "settings"> "store" > tap the
      Apple ID and choose "sign-out" > Tap "sign -in" > "use existing Apple ID" and you
      should be asked to verify your security code for the credit /debit card and NOT the
      security questions.
d. At this time you can remove the card by going back in to edit the payment info and
      selecting "none" as the card type then saving the changes by selecting "done". You
      should now be able to use your iTunes store credit without answering the security
      questions.
It's working for me ...I just have to put in my 3 digit security pin from the credit card I am using.

You need to either reset the rescue email address's password(check the site's help for instructions) or ask Apple to reset your security questions; ways of contacting them include clicking here and picking a method for your country, phoning AppleCare and asking for the Account Security team, and filling out and submitting this form.

How to reset security questions when I didn't use one they keep asking me

I keep getting asked my childhood nickname as one of my security questions and I never had one and didn't pick this question, so how do I get rid of it when it keeps asking me this question and telling me I got it wrong? Then it wont let me go any further to change it.

If you can remember their current answers then you can login here and change them : https://appleid.apple.com
If you can't remember their answers and you have a rescue email address (which is not the same thing as an alternate email address) set up on your account then you can try going to https://appleid.apple.com/ and click 'Manage your Apple ID' on the right-hand side of that page and log into your account. Then click on 'Password and Security' on the left-hand side of that page and on the right-hand side you might see an option to send security question reset info to your rescue email address.

The HT5312 page that you posted from has instructions for how to reset them i.e. if you have a rescue email address (which is not the same thing as an alternate email address) on your account then steps 1 to 5 half-way down that page should give you a reset link.
If you don't have a rescue email address (you won't be able to add one until you can answer your questions) then you will need to contact Support in your country to get the questions reset.

When they've been reset (and if you don't already have a rescue email address) you can then use the steps half-way down the HT5312 page that you posted from to add a rescue email address for potential future use
