Call AppleCare, tell them you just want to check the lock status, and give them the model and serial number.

OK, call AppleCare in the UK...no one else can help you...be prepared to give them the phone's IMEI number & SN. They can verify whether your phone is carrier locked or not. Tell them you asked for an unlocked iPhone, that there was no sim in the phone or carrier name on the box or receipt. They can fix this remotely, by unlocking your phone, if it is in fact carrier locked & you were given this phone by mistake.

Purchased a new, unlocked iPhone 4 directly from apple.com. We're going to be using it for in-house testing, and we don't want/need a service plan.
But we can't activate the phone without a SIM card. The only decent advice we've gotten is to activate it with someone else's iPhone 4 SIM card. But we'd need to do the same thing for any iOS update.
When I visited AT&T, they said I couldn't get an iPhone 4 SIM without a contract. I'm not entirely sure that employee was correct.

You can purchase an unlocked iPhone, without contract, directly from Apple online or in any Apple retail store. Then get a cheap T-Mobile sim & activate the phone. Any T-Mobile store will be glad to help. AT&T wants ALL iPhone users under contract. While it is possible to establish service on a post-paid basis, AT&T does not like to do this for iPhone users. Since your phone is officially unlocked, you don't have to deal with AT&T.

Stedman1 is right. Go to https://expresslane.apple.com and follow the prompts for iPhone. That's how you can contact Apple either by phone or by Genius Bar appointment for service or replacement. Your phone has a one-year warranty on it, so none of the 5s are out of warranty yet.
By the way, you don't need to restore as new multiple times. That doesn't do anything different than the first time.
