If your talking about the actual number that has been scratched off then toss it, it's now just a piece of plastic with some pretty writing on it. That number is what gives it value, with out it Apple wouldn't know what to do with it.
Try here for more answers possibly.
You might try e-mailing Apple directly and asking them as this is just a user to user forum, Apple just hosts it, they don't participate.

When you say iTunes, what exactly - Just a back up or were you syncing them to Outlook/Address Book app?
If it's a backup then you can only do a full restore to get them back (that's the point of a backup!). If you were syncing them (different to a back up) to Outlook (windows)/Address Book (Mac) then it's just a case of setting the sync back up on the new phone in iTunes.
I suspect you did a back up from reading your post though..

If you redeemed the card prior to losing it, then the balance is in your iTunes Store account and the card is no longer needed. If you didn't redeem the card before you lost it, then sorry but just like cash, it's gone.

Once pictures are moved from your camera to your iPad (using the camera connection kit), how do you get the photos off of the iPad? They do not show up in my photo stream or my iCloud. I am able to see any photo I took on the iPad using the iPad camera (in iTunes, photo stream, or when I open windows explorer and browse the iPad director), however I cannot see any photos transfered from my camera to the iPad. The camera connection kit must have put my photos in a directory I cannot get to and cannot sync.

You cannot call anyone. You contact them as everyone else does: http://www.apple.com/support/itunes/contact/
You delete the app as you do any other app. Hold an icon until they wiggle. Tap the "x". This is covered in the manual.

Try cleaning the lens and see if that will restore functionality to the DVD drive. Use a DVD lens cleaning disk, if you have a can of compressed air, shoot some into the slot or wrap a fine microfiber cloth (eyeglasses cleaning cloth) around a business card and insert it gently inside the slot.
If no success, make an appointment at an Apple store genius bar and get a free diagnosis from them.
