Options for when an iOS device gets locked because of forgotten password:
Restore (and reset password) on your device by connecting it to the last computer to which it was connected:
iTunes: Backing up, updating, and restoring iOS software -http://support.apple.com/kb/HT1414
If you cannot connect it to the computer to which the device was last connected you will have to use recovery mode to completely reset the device, losing all data:
iOS: Unable to update or restore - http://support.apple.com/kb/HT1808 - recovery mode (e.g., cannot connect to computer last used to sync device, iTunes still asks for a password)

    If she's forgotten the passcode to unlock the iPad then she will need to connect the iPad to the computer that she normally syncs to and she should then be able to reset the iPad and restore/re-sync her content to it (http://support.apple.com/kb/HT1212) - she may need to put the iPad into recovery mode in order to be able to reset it : http://support.apple.com/kb/ht1808
    If you do it via a different computer then :

    Connect your iDevice to your Computer and open the iTunes program. You will need internet access and a Apple account. If not, this will not work. 
    2Turn on your iDevice. Now hold down the Sleep/On/Off button and the Home Button at the same time for about 30 seconds.Skip the Turn Off Menu when it appears. Continue holding down the buttons.
    3If done correctly, your iDevice has turned off and then 1-3 seconds later, the Apple logo will came up, continue holding down the Power and Home Buttons. In approximately 20-30 seconds after it shows the Apple logo, it will display a "Connect to iTunes" image.
    4Now iTunes will show the iDevice and say: "iTunes has detected a iDevice in recovery mode. Please restore it before using it with iTunes."If you did not connect the iDevice to iTunes before you continued with the steps then this will not work. Also, iTunes doesn't distinguish an iPhone with an iPod in this message.
    5Click on restore on iTunes.Restoring the iDevice means to delete everything that is saved to the device. Including the password. It deletes everything because the passcode is designed to protect the owner's privacy. Thus, deleting everything is the only way to reset it.
    6After the download is complete, iTunes will automatically install it to your iDevice.
    7Sync your iDevice (Or use the backup if you did back it up) to iTunes and you should be back to normal. No password necessary!

    Saw this solution on another post about an iPad in a school enviroment. Might work on your iPad so you won't lose everything.

    ‘iPad is disabled’ fix without resetting using iTunes
    Today I met my match with an iPad that had a passcode entered too many times, resulting in it displaying the message ‘iPad is disabled – Connect to iTunes’. This was a student iPad and since they use Notability for most of their work there was a chance that her files were not all backed up to the cloud. I really wanted to just re-activate the iPad instead of totally resetting it back to our default image.
    I reached out to my PLN on Twitter and had some help from a few people through retweets and a couple of clarification tweets. I love that so many are willing to help out so quickly. Through this I also learned that I look like Lt. Riker from Star Trek (thanks @FillineMachine).
    Through some trial and error (and a little sheer luck), I was able to reactivate the iPad without loosing any data. Note, this will only work on the computer it last synced with. Here’s how:
    1. Configurator is useless in reactivating a locked iPad. You will only be able to completely reformat the iPad using Configurator. If that’s ok with you, go for it – otherwise don’t waste your time trying to figure it out.
    2. Open iTunes with the iPad disconnected.
    3. Connect the iPad to the computer and wait for it to show up in the devices section in iTunes.
    4. Click on the iPad name when it appears and you will be given the option to restore a backup or setup as a new iPad (since it is locked).
    5. Click ‘Setup as new iPad’ and then click restore.
    6. The iPad will start backing up before it does the full restore and sync. CANCEL THE BACKUP IMMEDIATELY. You do this by clicking the small x in the status window in iTunes.
    7. When the backup cancels, it immediately starts syncing – cancel this as well using the same small x in the iTunes status window.
    8. The first stage in the restore process unlocks the iPad, you are basically just cancelling out the restore process as soon as it reactivates the iPad.
    If done correctly, you will experience no data loss and the result will be a reactivated iPad. I have now tried this with about 5 iPads that were locked identically by students and each time it worked like a charm.

    Welcome to the Apple community.
    You can only unlock your device with the code that was entered into it when it was locked. Your only other alternative is to restore it to factory conditions and start again.

    To reset the code, you will need to select the iPod in iTunes and click Restore.
    This will erase the iPod and restore the iPod OS (and remove the screen lock and volume lock codes).

    1. Data will be lost if you do not have backup

    Yes.  Connect the iPod to your iTunes library in Recovery Mode and restore it as a new iPod.  See this article for instructions on how to place your iPod in Recovery Mode.
