HT201303 I have found out to my expense, that my 5 year old has been purchasing various app add ons with out having to input a password. It appears that if the password has been entered previously, pending timing you can ( a 5 year old can) make purchases

Is there a preference were password must be entered each and every time a request to purchase is made, regardless on timing since password previously entered.

All purchases are considered final, but you can try contacting iTunes support and explain what happened and see if they will refund or credit you : http://www.apple.com/support/itunes/contact/ - click on Contact iTunes Store Support on the right-hand side of the page, then Purchases, Billing & Redemption
To try and stop it happening again, you should be able to turn off in-app purchases via Settings > General > Restrictions > In-App Purchases 'off', and (depending upon what iOS version that you have on your iPad) you might also be able to set 'Require Password' to 'Immediately' on the Restrictions screen.
