If you already have an Apple ID and you want to create a new one without a credit card you have to make sure that you are signed out of your current Apple ID in Settings>Store>Apple ID - tap your ID and sign out.
And.... if you already have an Apple ID you can not use the email address from the current Apple ID for the new one. If you try to do that, you will get a message that your email address is already in use. So you will need another email address as well.
Like Skydiver, I use gift cards exclusively now. I did set up my Apple ID with a credit card way back, but I have not used it for an iTunes purchase in ages.
