HT201328 Unlock worked but then some weeks later it reverted to being locked again...?

I received a replacement iPhone 4 from Apple UK as I water damaged mine.  I followed the restore process via iCloud whilst I was in the store and the phone was working properly.  Some months later I discovered that it didn't appear to be unlocked, yet my previous one was unlocked by the carrier.  I visited the Apple UK store and spoke to one of the chaps there and they informed me that they could see the "unlock" was pending and I needed to perform a restore which I duly did on 29 Dec following the instructions in HT5014.  The restore and unlock worked as my other sim worked - it's from DigiMobil in Spain and I only use it when I go abroad.  I then switched my sim back to my normal home carrier, Vodafone UK, and that worked too so all looked perfect.  However, some weeks passed and I put my holiday DigiMobil sim in the phone and now it can't get a signal so it appears to be locked again.  I have just done another restore today, 13 Jan and the DigiMobil sim now works again.  I also put my Vodafone UK sim back in and that worked too, however when I put the DigiMobile sim back in again, it has failed to get a signal again!
However, my question is: What needs to be done so that I don't need to continuously keep restoring my phone when I want to switch between sims?
Other facts which may help:
- I know the DigiMobil sim works as I've tested it in another handset.
- I don't recall ever getting a message stating that my unlock was successful,

I have subsequently been able to get a signal with my DigiMobil, it just took about 20 mins to get it after searching and no service.  I can also get a signal with my Vodafone UK so it does appear to be working.
However, before the erase and restore today, I waited all day yesterday and only ever received no service for the DigiMobil sim.  So let's see if it's properly fixed or whether I'll be back in a few weeks after having performed another erase and restore...
