All content and data is forever associated with the Apple ID you used to download it. You should have reset the password for the original I'd.

Go to http://appleid.apple.com and click 'Find your Apple ID'. Follow the instructions and hopefully you will be able to identify your ID.

Content (apps, music, ibooks, films etc) is tied to the account that originally downloaded it, so if the iBooks app was downloaded using the account that you used to share with your ex then only that account can download updates to it. As it's a free app you could delete it and re-download it under your own account - and then re-download your ibooks into it (or copy the books to your computer's iTunes first via File > Transfer Purchases and then sync them back to the app).

If the old ID is an earlier version of your current ID, temporarily recreate the old ID by going to https://appleid.apple.com, click Manage my Apple ID and sign in with your current iCloud ID. Click edit next to the primary email account, change it back to your old email address and save the change. Then edit the name of the account to change it back to your old email address. You can now use your current password to turn off Find My iPhone, even though it prompts you for the password for your old account ID. Then save any photo stream photos that you wish to keep to your camera roll. When finished go to Settings>iCloud, tap Delete Account and choose Delete from My iPhone when prompted (your iCloud data will still be in iCloud). Next, go back to https://appleid.apple.com and change your primary email address and iCloud ID name back to the way it was. Now you can go to Settings>iCloud and sign in with your current iCloud ID and password.

No. purchases are forever tied to the apple ID used to buy them. Delete them and buy your own copies.

Did you create a new account or update the old one ? If you created a new account then any content that you purchased/downloaded via the old account will remain tied to that old account, and only that old account can download updates to its apps.

See Recover your iTunes library from your iPod or iOS device. But you'll still need the password.
Once you have the computer authorized to use the account she could change the password again to stop you buying apps on her card (assuming it's not on yours!). It would lock you out of upgrading them too but they should work unless she uses the deathorize all feature.

It sounds like your device might be in Activation Lock. Is it asking your to enter in an Apple ID & password, but only showing you the first letter of the email address and then some dots? Example: j•••••@icloud.com?
If so, do you recognize that email? Do you have an Apple ID that fits the same format? If so, then go to www.cloud.com, log in with that Apple ID & password, and see if under Find My iPhone it shows this device. If so, turn off the device, then remove the device from the list.

Careless? Possible, but unlikely. Most likely the phone was stolen from the first owner. It would be a VERY careless iPhone owner who did not erase their personal information from a phone before selling it, and to do that it is necessary to disable Activation Lock. Small chance that you have been infected by Trojan.OSX.BASH/QHost.WB.A that was out for about a day back near the end of July. To be certain open Network Utility from your Utilities folder and do a ping for google.ca and google.com. Also do a locate for each. If the results of the ping show an IP address of 91.224.160.26 that confirms the infection. If the ping IP matches any of the locate IPs then your OK.
