Where are you located?
Just go and buy an iTunes gift card at any store in your country.
Then follow all the steps you did, but when it asks you for the credit card number, there shoujld be a GIFT CARD option which will let you load your account witht eh funds form the gift card without providing a credit/debit card #.

I followed the instructions 3 times
- on my daughters iPod Touch (old iPhone 4). My daughter purchased a book from i Books which I why I need to find a way to not associate a credit card with an Apple ID.
- on my iPhone 5
- on my Macbook Pro via iTunes running Mountain Lion
In all three cases the behavior was very similar. I was never presented with the "None" option. Most of the time I was presented with only credit card options and the last time I was presented with an option for a credit card or paypal.
How can there not be an option for a child to download free apps and books (age appropriate) without being able to purchase apps, games and books?

We cancelled our credit card and now, months later...my iPhone won't update any of the "Free" apps that I have, I don't want another credit card just so my iPhone will function. Is there a way to connect it to Paypal or anything else? It only gives me the "Visa, Mastercard, Amex" option on my phone and I can't alter payment methods from the AppleID.com site.

You created the account by selecting a free app in the store ? The screenshot that you've included isn't from the 'create Apple id' process, when creating the account I get the above on the 'provide a payment' part of the process
