If it's a purchase that appears on your purchase history (and not a temporary store holding charge which is made each time that you add or change your credit card details) and nobody else has access to your account and/or devices, then you can either try the 'report a problem' link from your purchase history to contact iTunes Support : http://reportaproblem.apple.com

How/where are you downloading the book ? If you've been gifted a book from a Kindle then you should be going into the Kindle app on your iPad and downloading it in there, if you are going into the iBooks app and downloading it from Apple then you will be charged - Kindle and iBooks are two different stores run by different companies (and ibooks from Apple can't currently be gifted).

thats stupid because if you are trying to get something is supposed to be "free" why the heck do we have to provide with a Credit Card info??? Thats dumb, what if we dont have a Credit Card???

You've checked the purchase history on your account via your computer's iTunes : log into your account on your computer's iTunes via the Store > View Account menu option, you should then see a Purchase History section with a 'see all' link to the right of it. Click on that and you should then see a list of your purchases.

If nothing shows on your iTunes purchase history (you don't have a Mac and have any purchases in the Mac App Store), and you only have the one account, then you should contact your card issuer.

What process did your daughter use, there is a specific format to redeem an iTunes gift card. The gift card must be redeemed using the Redeem link in the Quick Links on the right side of the iTunes Store in iTunes on a computer or the Redeem button at the bottom of both the iTunes and App Store apps in iOS. Redeeming the gift card then applies the value of the card as an available credit to the user's iTunes account. The credit is used first before any bank card also stored in the account. If the purchases use the entire credit and there is an additional balance due, it will be charged to any bank card stored in the account.

I upgraded my Icloud storage capacity to a higher tier and my Iphone does not reflect the change although my credit card was charged and the device is nor properly backup. How can this be resolved?
It seems to take up to a couple of days for the upgrade to take hold, at least that's the experience of some users. Give it 24 hours before contacting apple.

Yo Chris. I've been following your answers to different threads and quite frankly, you're very keen in answering everybody regarding this stupidity that Apple has brought upon its customers. Kudos to you. But the problem is, most of the time, in some devices, your solution doesn't work because the NONE option is not there. The same goes for me.
Meanwhile, I found a temporary workaround for the iPhone 4s, iPad 2 and iPod Touch 4th Gen.
Register a free Apple ID from the iCloud Settings.
Go to Settings>iCloud>Account>Tap Payment Information on the Storage Plan Category.
Fill in the other boxes and choose "None" on Credit Card Information.
That's it! Hope I helped those who still couldn't find the "None" Option.
Must be running IOS 5.0.1 though, else there would be no iCloud. -
My credit card was charged for two purchases in the amount of 14.95 each. I did not authorize these purchases. The purchase dates were 12/24 and 12/25.
