If the silver backing hasn't been scratched off, the chances that the card has been redeemed are very, very small. Whether or not the seller activated it will be impossible to know until you try to redeem the card.

The gift card field is optional, you don't have to fill it in - and as cards are not available in all countries (and they are country-specific, they can only be used in their country of issue) you might not be able to fill it in anyway.
If it's a new account, and you didn't use the instructions on this page when creating it :http://support.apple.com/kb/HT2534, then you will need to enter payment details on the account before you will be able to download any item from the store with it.

Whether you can get an iTunes gift card depends upon what country that you are in - they are not available in all countries and they are country-specific, they can only be used in their country of issue and only onto accounts that are based in that country. If they are available in your country then you might be able to get them from Apple, or from electrical stores, supermarkets etc

My account's balance shows at the top left of the store's page on my computer's iTunes, and by logging in to view my account on it (via the Store > View Account menu option). On my iPad and iPhone it shows at the bottom of, for example, the Featured tab in the App Store app under my account id (or you can view your account via Settings > iTunes & App Stores and it should show on the screen that you are taken to)

You can buy it (usually for the same or less amount of money as itunes gift card costs) from Apple stores and other online stores (unfortunately not everywhre in the world yet).
