1. Make sure software is up to date
2. Make sure FaceTime is enabled; Settings>FaceTime
3. Make sure Date and Time is correctly set; Settings>General>Date and Time>Set Automatically>On
4. Make sure Push Notification is enabled
5. Make sure phone number or email address is correct
6. Hold the Sleep and Home button down (together) until you see the Apple Logo

Could be anything from a chargeback, or your card is junk depending on what it is. Using the word debit card isn't realy descriptive, and can many mean many, many different things.

I have purchase some applications from app store using my VISA DEBIT card. Apps downloaded successfully. After download when I want change my payment method again to none, it says “enter a new VISA CARD to clear outstanding. I am entering my VISA DEBIT card no. but it says again to enter new card no.
When I check my Debit card statement there no payment made to apple, but apps downloaded on my iphone.
And when open itunes and want download more apps it say to enter CARD no.
Please help me.

Create a NEW account using these instructions. Make sure you follow the instructions. Many do not and if you do not you will not get the None option. You must use an email address that you have not used with Apple before.

How do I get the app store to use my debit card?

Okay so when I first got my Mac Book it was linked to my Dad's account so therefore everything comes out of his account whenever I purchase an App from the App Store. I did try linking my own debit card to my apple ID by going to where it says you can change your billing information and adding my card information. So after that I tried purchasing something but it didn't work because I checked my online banking and it is still the same amount it was before I purchased it. Does anyone know what else I can do? I would really like to switch my apple ID to my account. My debit card is a Visa if that matters.

App store not accepting my debit card. But I'm still being charged?

So Ive been trying to buy this app for a while now so I bought a $15 app store gift card. The app is 14.99 so I apparently forgot about tax. So I try to put my debit card info in (paypal) and it keeps getting denied. Next I checked my paypal acount to see that I have a few 1 dollar charges pending and I'm not really understanding whats going on. This even happened after havin to "identify" myself before downloading a free app. So whats going on? BTW Im on an iPad 3 ios 5

What do you mean by 'refusing from the system', if you are getting an error message then what does it say ?
For a credit card to have a chance of being accepted it needs to be registered to the same name and address (including format and spacing etc) that you have on your iTunes account, and have been issued by a bank in the country where you and your iTunes account are. If it is and if you are getting a 'declined' message then you could check with the card issuer to see if it's them that are declining it, and if not then try contacting iTunes Support (these are user-to-user forums) and see if they know why it's being declined : http://www.apple.com/support/itunes/contact/ - click on Contact iTunes Store Support on the right-hand side of the page, then Purchases, Billing & Redemption
If when trying to download you are getting a message about a 'problem with a previous purchase' then that implies that iTunes wasn't able to collect the money for your last purchase. If that is the case then you won't be able to download anything else (including app updates) until you've paid off what you owe : Pay an unpaid balance in the iTunes Store - Apple Support
If you aren't getting the 'problem with a previous purchase' message then try logging into your account (not when trying to download something) and see if you get the 'none' option so that you can remove the card's details and be able to download updates to your apps : Change or remove your payment information from your iTunes Store account (Apple ID) - Apple Support

Back up all data.
Unlock the Network preference pane, if necessary, by clicking the lock icon in the lower left corner and entering your password. Cllck Advanced, open the DNS tab, and change the server addresses to the following:
          8.8.8.8
          8.8.4.4
That's Google DNS. Click OK, then Apply.
In Safari, select
          Safari ▹ Preferences... ▹ Privacy ▹ Remove All Website Data
and confirm. If you’re using another browser, empty the cache. Test. Any difference?
Notes:
1. If you lose Internet access after making the above change to your network settings, delete the Google servers in the Network preference pane, then select the TCP/IP tab and click Renew DHCP Lease. That should restore the original DNS settings; otherwise restore them yourself. Remember that you must click Apply in order for any changes to take effect.
