Was the iTunes card issued in the same country as your account is based (they are only valid in their country of issue) ? If it was and this page doesn't help then you will need to try contacting iTunes Support (you will probably need to give them images of the front and back of the card, and possibly its receipt) : http://www.apple.com/support/itunes/contact/ - click on Contact iTunes Store Support on the right-hand side of the page, then iTunes Cards And Codes

If you've forgotten your answers to your security questions then you can get them reset : If you forgot the answers to your Apple ID security questions
If you have a credit card on your account then its details have to complete - and asking for their details is also a way of checking that you have a valid billing address in that country. As long as you are buying content for yourself and not trying to gift content (you can't use your balance for gifting) then the purchase price (plus any applicable taxes) should be deducted from your balance. I had redeemed 2 $50.00 iTunes gift cards a few months ago. The last time that I checked my iTunes balance, it was $100.00. I logged in to iTunes yesterday and the $100.00 balance isn't showing up. Why would it be missing? Would that happen if I changed my apple ID and password recently? Or would that happen if I de-authorized 2 devices? Would that happen if I deleted my music from my iPhone? Could someone hack into my iTunes account and take my money? Or maybe Apple charged me for something and debited my iTunes account? Check your account's purchase history : See your purchase history in the iTunes Store - Apple Support
If nothing shows then try logging out and back into your account on the computer/device that you are using.
Neither updating your account id and/or password, nor deauthorising your account, nor deleting content from a device or computer's iTunes library will affect your balance -

My husband and I recently decided to acquire a new Credit Card and keep it off the internet

this problem. Would be grateful if anyone has any info.For apps, sorry but no. Canadian users cannot, for reasons not well explained, cannot purchase software from the iTunes Store via prepaid cards. You'll need to enter in a credit card, at least temporarily, to pay off the balance.

Return it or give it to someone in the U.S., then buy one in Canada when you get home.

my iTunes gift card is not working
In that case the procedure assuming it is a PAYG phone is to get an o2 PAYG sim, put £20 on it and then make a few calls making sure you leave £15 on the sim to pay for the unlock. Then there is an online unlock request form on the o2 site just fill this in. It takes between 1-14 days and you get a message from o2 telling you how to complete the official unlock through iTunes.
I got my o2 unlock in nov 09 so this is not just opinion. -
