When you say you just purchased Formscentral exactly what do you mean? Formscentral has not been sold as a stand alone product in a while due to the decision to close the service. If you got Formscentral as part of a Acrobat subscription then you need to know that that part of the subscription will go away partially in June and fully in July. If you have indeed managed to purchase Formscentral standalone recently can you please give me the information about your purchase such as the date of purchase and link to the site from which you purchased. Refunds are available to customers who are not able to use the service though their contract period.

Contacting iTunes Support isn't all that hard. Just go to the Support tab above, click the Get Started link in the Contacting Apple Support area in the middle of the page, and follow the on-screen instructions.

I ordered a movie on iTunes back in April, the movie didn't finish downloading, I got a message that there was an error, so a few days later, I downloaded it again. This time I used my husbands bank card, which has a different number, but is connected to our joint checking account. I noticed the duplicate charge for the one movie. Did you end up buying this twice because you were trying to download the app a second time? If so, you should:
Select the store on the left side of iTunes.
Click on Purchased on the right side under Quick Links.
You can redownload your available previous purchases.

It is possible that you accidentally double clicked the order in some way, some how processing it twice. Clearly you don't need two copies of the same magazine and it is unlikely Apple will not refund you.
On the receipt there should be a section asking you to report a problem. I suggest you do just that and the matter will be resolved.
