    Is there some reason you cannot provide the exact title here as was  requested?
    If the book is not in the Apple store, you will have to get it from another one, such as Amazon, Nook, Kobo, Google, Sony.  Each of them has apps to read the books  you get from them.

    There is no automatic way to do this because any purchases made with your account are assumed to be authorized by you.  If this involved account hacking you can report it to iTunes Store but purchases made with a credit card should be reported to the card fraud department.

    You will need to contact iTunes Store Support.
    You may want to consider turning off In-App purchases in the Restrictions:
    Settings > General > Restrictions > Enable Restrictions > Create a passcode > verify passcode > Turn off In-App Purchases.

    Also you need to go to settings, general, restrictions, and set the restrictions to prevent in app purchases, it will have a passcode on it .

    To try and stop it happening again you can turn off in-app purchases via Settings > General > Restrictions > In-App Purchases 'off', and depending upon what version of iOS that you have on your iPad you might also be able to set 'Require Password' to 'Immediately' on the Restrictions screen (the default is a 15 minute period during which it doesn't need re-entering).

    Keeping current plan, but downgrading when it ends:
    Settings>icloud>Storage & Backup>Change Storage Plan.  Look at downgrade options.  There you can choose the plan you want when the current one ends.

    You should always sync them to your computer:  File>Transfer Purchases and you should alwys maintain a backup copy of your computer.
