Disabled
Place the iOS device in Recovery Mode and then connect to your computer and restore via iTunes. The iPod will be erased.
iOS: Wrong passcode results in red disabled screen
If recovery mode does not work try DFU mode.

If you restore from iCloud backup the apps will be automatically downloaded. If you restore from iTunes backup the apps and music have to be in the iTunes library since synced media like apps and music are not included in the backup of the iOS device that iTunes makes.
You can redownload iTunes purchases by:

Since you say you also forgot your security questions, I wonder if you just purchased this iPhone second hand. If so, it is likely Activation Locked and would therefore be of no use to you.

If you are unable to remember your password, security questions, don’t have access to your rescue address or are unable to reset your password for whatever reason, your only option is to contact AppleCare(or Apple ID Support), upon speaking to an operator you should explain that your problem is related to your Apple ID, this way you will not be charged for assistance, even if you don’t have an AppleCare plan. -

I can't sign into iCloud because my Apple ID doesn't work there. For some reason it gave me a .mac email and I don't remember the password. That Apple ID is stored inside each of those applications. If you can’t get back into it, you’ll need to delete and redownload all of them, which may require repurchasing paid ones.

Recovery Mode
1. Disconnect the USB cable from the device, but leave the other end of the cable connected to your computer's USB port.
2. Turn off the device: Press and hold the Sleep/Wake button for a few seconds until the red slider appears, then slide the slider. Wait for the device to turn off.
3.While pressing and holding the Home button, reconnect the USB cable to the device. The device should turn on.
4. Continue holding the Home button until you see the "Connect to iTunes" screen. When this screen appears, release the Home button. iTunes should alert you that it has detected a device in recovery mode. Click OK, and then click Restore to restore the device.
Note: Data will be lost. -
There are quite a few songs, videos, television shows, and movies I purchased years back, well before the cloud with two different user id's that I don't remember the passwords for, and when I try and get to the security questions, it says the information doesn't match. I'm ****** because I'm sure they will have some convenient excuse for not getting me out of this mess with my new laptop, but as long as my old one works, I can still access the previous library, where EVERYTHING plays. How can the new laptop not be recognized when I had all of the information transferred from the old laptop? Now I have to hope my old laptop doesn't crap out for a few years longer!!
I have the same problem - it is maddening. I rely on this iPad for work so this is not just an annoyance! The above solutions of changing the appleid on the device or on the website do not work.
The old email address no longer exists - I haven't used it in a year probably and I no longer have the account. I logged into the appleid website and there is no trace of the old email address so there is nothing that can be deleted or changed there. On the iPad there is no trace of the old email address so nothing can be deleted there either. I have updated the iPad software and the same problem comes right back. Every 2 seconds I am asked to log in using the old non-existent email. The device is currently useless.
The only recent change to anything was the addition of an Apple TV device, which was set up using the correct login and password.
Does anyone have any ideas? The iPad has been backed up to the iCloud so presumably it now won't recognize the current iCloud account? So restoring may notbe an option? -
Please help...I have one than more IDs. I cant remember the password or the email account that I purchased most of my music under. My computer is authorized under my current ID but not my old one. When I try to enter the answers to my security questions I get an error message that my birthdate is wrong.... any thoughts?...
If you can't reset the password by email, and you put the wrong date of birth on your account, then try contacting Support in your country and see if they can reset the password for you (they should be able to if they can confirm your id and that it's your account).

If you are the original owner of the phone, you can force it into recovery mode to restore it as explained here: If you can't update or restore your iPhone, iPad, or iPod touch. You will then go through the setup screens on the phone to reactivate it. This will require entering the ID and password that were used to activate the phone originally. -
