You will need to restore your device as New to remove a Restrictions passcode. Go through the normal process to restore your device, but when you see the options to restore as New or from a backup, be sure to choose New.
You can restore from a backup if you have one from BEFORE you set the restrictions passcode.

iTunes match is a subscription that you have in when you were still in the UK. According to the terms & conditions you'll need to wait for the susbcription to end before you can change your country to iTunes Store USA.

Maybe I should have set it up as new? How do I change that?

If you don't transfer the backup for your other iPhone or leave it as is, you must set up and transfer everything manually after restoring your new iPhone as a new iPhone or not from the backup.

See http://support.apple.com/kb/HT5787. (Your iCloud ID is also an Apple ID.) If you don't have access to your old email account and can't use security question authentication to reset your password, you'll have to do one of the following to turn off Find My iPhone so you can chang the iCloud account on your phone:
If you still have access to your old email address, go to https//appleid.apple.com, click Manage my Apple ID and sign in with your iCloud ID. Tap edit next to the primary email account, tap Edit, change it back to your old email account and verify it. Then edit the name of the account to change it back to your old email address. You should then be able to turn off Find My iPhone with your password. Then go to Settings>iCloud, tap Delete Account and choose Delete from My iDevice when prompted (your iCloud data will still be in iCloud). Next, go back and change your primary email address and iCloud ID back to the way it was. You will then be able to go to Settings>iCloud and sign in with your current ID and password to reconnect to your iCloud account.
If you don't have access to your old email address, you will have to contact Apple to get them to reset the password for it so you can disable Find My iPhone and sign into your iCloud account. You will have to prove your identity to them in order to do so. Settings-icloud-scroll to the bottom, select delete, if you want to keep the content select keep, otherwise it'll remove it. Then once it is done add the account information for the account you want on that phone. Your apple ID in settings-store will remain the same and each device can still purchase/use content from that account.

The new iTunes 12 Get Info dialog lacks right-click context menus (at least for now) but it still accepts most standard keyboard shortcuts and drag & drop actions.
Delete Artwork
On the artwork tab of Get Into, tap the image in question, use Backspace or Ctrl+X. Delete doesn't work.
Or, tap the image at the top left of Get Info, this time use Backspace or Delete, it is Ctrl+X that doesn't work.
Add Artwork
Select artwork, press Ctrl+C to copy, tap the centre of the Artwork tab, press Ctrl+V to paste.
