If you don't know the passcode you'll have to force it into recovery mode to restore it, as explained here: http://support.apple.com/kb/ht1808.  Then restore it to an existing backup, if you have one.

    Restore from iTune Backup
    1. Settings>General>Reset>Erase all content and settings
    2. You'll be asked twice to confirm
    3. You'll see Apple logo and progress bar
    4. You'll see a big iPad logo on screen

    8. Select Network and input Password>Join

    21. Copying Apps back to iPad (computer)
    22. You'll see Loading/Installing/Waiting below the Apps (iPad)
    23. Sync Music/Podcast/Movies to iPad (computer)
    24. Sync completed (computer)

    the fact is that someone stole my Mac about 1 and a half years ago! so the day after i realise my mac wasn't in the my house i immediately blocked by find my iphone with the hope to find but i didn't get nothing, then after 1 year i received a message telling my that my Mac was successfully blocked and my mac used to call "Kevin Gr. MacBook air" and the message i got from apple said that "Dou MacBook air" was successfully blocked. that time i realised who was the one who stole my Mac a "friend" :/ he was the only one that i know that uses that nickname for everything! well i didn't did much because i wasn't that sure that he stole it! but one day in a party in his house well not a party but all friends hanging out, they decided to go to the disco so i said that i was very tired and i managed to stay at his home, so when they left the house i decided to search for my thing, and after 30 min of searching i finally found all the things they stole :/ and my mac was there!   so I immediately took all my thing and left the house! i called him telling him that i found all my things and ill took them home! he was stunned for a moment and then he said sorry for everything, but all that things he was going to buy! but he knew it was our stolen stuffs and blah blah blah
    well now i wanted to open my computer and its locked with the 4 digits code that i put a year ago and i don't remember, there is a way to unlock it? or i just need to reinstall the OS!

    You are not inputting the correct password. Please visit
    support.apple.com/kb/PH11117
    and learn how to reset your password. If you still cannot get in it's time to take it into your local Apple Store or AASP.

    Welcome to the Apple Community.
    You can only locate your device when it is logged into iCloud and 'Find My Phone' is enabled, additionally the device will need to be switched on and connected to a wifi or cellular network.
    Unfortunately, you cannot activate iCloud or 'Find My Phone' remotely.

    Do you mean iTunes library?
    iTunes account is what you use to make iTunes purcahses.
    iTunes library is what is on your computer.
    It says my phone can only be set up woth one itunes account. How do I remove the previous account?
    No, it tells you it can only be sync'd with one iTunes library.
    Simplest way is to copy the entire /Music/iTunes/ folder (thisis the iTunes library) from old computer to new computer.

    Here's an interesting bit: iTunes will let me restore my iPod touch from backup: it's still running iOS 4.3.3. I figured as much... there's backups showing in the Preferences -> Devices list for both devices (and even my old, long gone iPod touch 2G), but my iPhone (on iOS5) isn't giving me any such option. I remember I had to downgrade before to get the backup restored (something went haywire in the setup assistant, and I couldn't restore a backup from iTunes there)... If it was possible to go to iOS 5.0 rather than 5.0.1 I would just do the same thing again, but iOS 5.0.1 isn't an option. I attached a screenshot. Hopefully Apple can get this nonsense sorted out; I think iOS 5 might be the root of this, somehow.

    If you do it only for your user, you can try resetting permissions > http://osxdaily.com/2011/11/15/repair-user-permissions-in-mac-os-x-lion/
    If this doesn't work, open Disk Utility from Recovery (if you have OS X Lion or Mountain Lion, pressing Command and R keys while your Mac is starting) or the Mac OS X DVD (if you have Snow Leopard or older, pressing C key while your Mac is starting), select Macintosh HD in the sidebar and repair permissions.
    If this doesn't work, reinstall Mac OS X

    You can can always unlock/use the device again by placing the iPod in recovery mode and restore via iTunes.

    I purchased a phone from someone I do not know very well because mine suffered water damage and I do not want to use my Verion upgrade until the iPhone 6 is released.
    He reset the phone but of course did not disable Find My iPhone. The guy is nice enough and tried to give me his log in information but could not remember it. He also used vague answers for his security questions, so I have not been able to unlock his account. I have tried to get him to call Apple himself, but he has since stopped dealing with me (I've probably driven him crazy, but he doesn't understand that what he would need to do is explain his situation and it would be relatively simple to convince Apple to reset his information).
    My question is this... If I wrote somthing out for him to sign stating that he sold the phone to me and cannot login to disable the application, will an Apple agent be able to reset it for me? Is there any way around this?

    You can't. Get your money back. It's useless to you. Nothing he could "sign" would do you any good. The only way Apple will intervene is if you have the original proof of purchase from an authorized source. If he won't or can't take care of the problem and won't give you a refund, turn the phone over to the police and give them all the information you can on the seller. It's likely it's stolen if he can't remove the lock.

    I have had 3 iphones stolen the last 5 months.
    Finally I got a ping from one of the stolen iPhones. I could follow it for almost 2 months. Since I am in a country were the police wont do anything it does not help me that I have name/address of the thief.
    But this strange thing happened last day/last ping I got from my stolen iPhone:
    Somehow the device is removed from my "find my iphone" list and/or the device have changed name.
    The device was locked with passcode/touch ID.
    I have changed password AppleID 3 times since the phone was stolen (well: The police wanted my password/AppleID to track the phone. Somehow the thief got this information and I found out when my address book had 150 new entries from the thief).
    I managed to change password and lock the phone remotely after that incident.
    To bad I cant ask Apple any question.
    How can a device be removed from my account? (I have not got any warnings that someone have logged into my iCloud account from a new device/location)
    How can a thief change the name of a stolen iPhone that is locked with touchID/Password/passcode? (Iphone6).  Is there still DFI attacks that can wipe the phones?
    How can I check if my account is hacked?
    Why cant I remotely detonate my iPhone? The battery would be a nice little bomb making the phone worthless?
    (and F lazy Russian tourists police that just ask "what should we do" when I provide address/name/video of a person that have stolen stuff worth over 5000 dollars including iphones/Ipads)

    Take a look at the instructions below for what to do when you sell your iPhone.  If you have followed all of these steps, the new owner should contact his/her carrier to activate the phone.

    With iOS 7 or later, Find My iPhone includes a feature called Activation Lock, which is turned on automatically when you set up Find My iPhone. Activation Lock makes it harder for anyone to use or sell your iPhone, iPad, or iPod touch if it’s ever lost or stolen. 
    With Activation Lock, your Apple ID and password are required before anyone can:
    Turn off Find My iPhone on your device
    Sign out of iCloud on your device
    Erase and reactivate your device

    The most anybody will be able to tell you is that it was somebody who knew (or guessed) the Apple ID and password you use for iCloud, so he/she appeared to *be* you when logged into Find my iPhone. (If you are using Family Sharing, it could be somebody else in your family group or somebody who knew or guessed their iCloud Apple ID and password.)
