How do I restore my songs from icloud to my pc and iphone?

Music purchased from the iTunes store can be redownloaded for free (in many countries), see http://support.apple.com/kb/ht2519.  Music from other sources isn't in iCloud, unless you subscribed to iTunes Match.

    You need to perform a reset (settings>General, scroll to bottom of screen).  This will delete everything on the device.  Then connect to iTunes and it will offer to perform a reset from either iTunes backup or iCloud.  Take it from there.

    It has moved.  Go to Edit > Preferences > Store, and check the box for "Show iTunes in the Cloud purchases."

    You'll need to restore the device from the settings menu, when it restarts the set up assistant will prompt you to restore from iCloud or as a new phone.

    Restoring an iCloud backup requires you to first erase your iPad.  If you have any newer data that you want to save you'll need to do that first.  Then go to Settings>General>Reset, tap Erase All Content and Settings, go through the setup screens on the iPad and when given the option, choose Restore from iPad backup.

    Documents do not get lost, they do get deleted. If you deleted - you need to look for local back up.
    If you "lost" you need to check if they still in the cloud at icloud.com
