    You don't. At least not for T-Mobile in the US. Sprint will unlock an iPhone if you are a current Sprint customer in good standing, but it can never be used on any US carrier other than Sprint. It can only be used on non US GSM carriers.

    If you want an iPhone to use on T-Mobile, buy one designed to work on their network.

    You don't. The Sprint iPhone 4 doesn't even have a SIM slot. There is no GSM radio in it. It is a pure CDMA phone. It will never work with any carrier other than Sprint.
    Buy a cheap disposable phone when you get to the country you will be visiting.

    In order to be factory unlocked, it is manufactured unlocked to any carriers at the factory.
    Phones that are locked cannot be "factory unlocked".

    You can't. Sprint phones can be unlocked ONLY for use on non US carriers. They can not be used on any US carrier other than Sprint. If you want to use a different carrier in the US, sell the phone and buy one you can use.

    To the best of my knowledge, Sprint will only unlock phones for international GSM roaming. Call Sprint for verification. Sprint phones will not work on Verizon's network.

    You are wrong.  That is not the only time you 'unlock' an iPhone.

    Unlocking a disabled iPhone refers to when the passcode has been entered too many times.
    There is a passcode lock function for iPhones.
    To unlock an iPhone that is passcode locked, you need to enter the passcode.
    Enter the passcode incorrectly too many times, and it becomes disabled.
    The only thing about the OP's post that even suggests this is an issue about carrier unlocking is the article he linked from, which is most likely explained because he seached for 'unlock iphone'.  First Google result is the article he linked from.
    But a carrier locked phone is not DISABLED.  You are ignoring that part of the OP's post, and focusing on the word 'unlock', which has multiple meanings.
    The most likely issue is a passcode lock on the phone, which was entered incorrectly too many times, and thus, the iPhone is disabled.

    Being 'locked' to a carrier has NOTHING to do with an iPhone being disabled.  Restricted to use on that particular carrier?  Yes.  Disabled?  No.

    No, Verizon will only unlock an iPhone 4S for international use. And only Verizon can legitmately unlock the phone.
