You should have been asked to set them when you created your iTunes account (unless the account was created prior to them being introduced), not when registering a device to your account.
Are you able to set them, or reset them via the Password & Security section on your account on http://appleid.apple.com ? You won't be able to reset them if you don't have a rescue email address (which is not the same thing as an alternate email address) on your account.
If you can't set/reset them then you will need to contact Support in your country to get the questions reset.

When they've been reset (and if you don't already have a rescue email address) you can then use the steps on this page to add a rescue email address for potential future use : http://support.apple.com/kb/HT5620

So basically I need to confirm my identity but I can't because I can't remember the answers to my security questions. I scheduled a meeting with Apple, to see if they can fix it. Nod I would just reset them from my rescue email, but I don't know what that email is and it's also no longer in use.

2. Fill out and submit this form. Select the topic, Account Security. You must
have a Rescue Email to use this option.
3. This is the only option if you do not already have a valid Rescue Email.

My daughter's itouch (registered to my apple account) is asking for answers to security questions that I never answered and it won't let her buy anything from the iTunes store. I tried a hard reset to original settings but it still asks for answers. How do I reset the security questions or get rid of them appearing altogether? I can purchase apps on my phone (same apple account) and it doesn't ask me for security questions.

Thanks for that but no joy, I got a call from an apple support agent and reset my id with security questions
but when i went back to purchase a song they were still incorrect and have now been locked out of my account, i have sent an email to apple to try and resolve, A couple of my friends are having the same problem since they downloaded the latest version of i tunes. -
I had just redone all my security questions and committed them all to memory. Then the very next time I tried to log in-in oder to 'verify' they made me make up brand new ones. How does this 'verify' if they are new ones? How do they expect you to remember the security questions if they make you reset them over and over again on the the same day?
Try:
- Reset the iOS device. Nothing will be lost
Reset iOS device: Hold down the On/Off button and the Home button at the same time for at
least ten seconds, until the Apple logo appears.
- Go to Settings>iTunes and App Store and sign out and sign back in
- Reset your Apple ID password

You might be able to re-enable it via this page : http://appleid.apple.com, then 'reset your password'
You might then need to log out of your account on your phone by tapping on your id in Settings > Store (Settings > iTunes & App Stores on iOS 6+) and then log back in so as to 'refresh' the account on it
For your security questions, if you can't remember them and you have a rescue email address (which is not the same thing as an alternate email address) set up on your account then try the steps half-way down this page : http://support.apple.com/kb/HT5312
If you don't have a rescue email address (you won't be able to add one until you can answer 2 of your questions) then you will need to contact iTunes Support / Apple to get the questions reset.

i can't purchase music on my itunes account anymore. it keeps asking for answers to security questions that i did not answer. i have purchesed all the music through my ipod but last time i tried it said " since this is the first time you are purchasing through you ipod you need to answer 2 security questions. how do i get past this. i have 1 guess left as to the possible answers. won't let me pick different questions.

If you have an existing Apple ID that you use with the iTunes Store or Mac App Store, you may be asked to provide some additional security information, including a rescue email address. When you provide a rescue email address, all future security emails, such as password resets from iForgot, will be sent to the address provided. The rescue email address you provide cannot be the same as your primary email address for your Apple ID. In most cases, your primary email address is the same as your Apple ID username.
