Welcome to the Apple Community.
Start here (change country if necessary) and navigate to 'Password and Security', reset your security questions using the link provided, you will receive an email to your rescue address, it has been reported that the email can take a while to arrive. Use the link in the email and reset your security questions.
If that doesn't help, you don't receive a reset email or you don't have a rescue address, you should contact AppleCare who will initially try to assist you with a reset email or if unsuccessful will pass you to the security team to reset your security questions for you.
If you are in a region that doesn't have international telephone support try contacting Apple through iTunes Store Support.

If you've been unsuccessful resetting your security questions by signing into your account at appleid.apple.com (manage my apple I'd) and the going to password and security, and have also tried contacting iTunes support and have not heard back then:

If you have a rescue email address (which is not the same thing as an alternate email address) set up on your account then go to https://appleid.apple.com/ and click 'Manage your Apple ID' on the right-hand side of that page and log into your account. Then click on 'Password and Security' on the left-hand side of that page and on the right-hand side you should see an option to send security question reset info to your rescue email address.
If you don't have a rescue email address (you won't be able to add one until you can answer 2 of your questions) then you will need to contact iTunes Support / Apple to get the questions reset (we are fellow users here on these forums).

Changing the rescue email address requires supplying two of the answers. They wouldn't be security questions if they could be bypassed without Apple verifying your identity.

I noticed a week or two ago that somebody had downloaded something on my apple account and it was not me and it was not on any device I own. I think I know the answers to these questions but I can never get it right, so I suspect that they were changed. I also noticed that my rescue email is not my own. I want to reset my security questions, and in order to do that I need to have access to my rescue email.
