If you have three security questions and a rescue email address
sign in to My Apple ID and select the Password and Security tab to send an email to your rescue email address to reset your security questions and answers (the steps half-way down that page should give you a reset link)
If you have one security question and you know your Apple ID passwordsign in to My Apple ID and select the Password and Security tab to reset your security question.
If you have one security question, but don't remember your Apple ID passwordcontact Apple Support for assistance. Learn more about creating a temporary support PIN to help Apple confirm your identity when you contact Apple Support.
If you can’t reset them via the above instructions (you won't be able to add a rescue email address until you can answer your questions) then you will need to contact iTunes Support / Apple in your country to get the questions reset.

I got some help from an apple assistant on the phone. It is kind of round about way to get in.
Here is what he said to do and it is working for me...
a. on the device that is asking you for the security questions go to "settings", > "store" >
tap the Apple ID and choose view"Apple ID" and sign in.
b. Tap on payment information and add a credit/debit card of your preference then select
"done", in the upper right corner
c. sign out and back into iTunes on the device by going to "settings"> "store" > tap the
Apple ID and choose "sign-out" > Tap "sign -in" > "use existing Apple ID" and you
should be asked to verify your security code for the credit /debit card and NOT the
security questions.
d. At this time you can remove the card by going back in to edit the payment info and
selecting "none" as the card type then saving the changes by selecting "done". You
should now be able to use your iTunes store credit without answering the security
questions.
It's working for me ...I just have to put in my 3 digit security pin from the credit card I am using.

Trying to download app on new iphone 5. I cannot remember answers to security questions. There is no rescue email option or any other option that I can find. Any suggestions? Help!
I just went through a similar situation. Here is what I did.
Note: Before starting your account MUST NOT be locked. If you have locked the account out, you must wait 8 hours for it to reset before proceeding.
Call 800 My iPhone. You can bypass the VRU by pressing 0 which takes you straight through to a support person. This person will not be able to help you directly but can put you through to account security support. Explain your situation and ask to be put through to the account security department.
Before account security can reset your security questions, they will need to verify your identity. They can do this by using any combination of 2 of the following methods
1) By correctly entering the answer to only 1 of your security questions.
2) By verifying a credit card associated with your apple id
3) By sending a verification code to your phone which you will then read back to them.
If you do not have a credit card associated with your Apple ID, add one prior to calling them. Go to Settings ... iTunes & App Stores ... then click on your Apple ID (top of screen)
Now, select View Apple Id from the popup menu, enter your password if prompted, click on Payment Information. You will now be able to add your credit card info. You can remove it later on by selecting 'None' on the same screen.
After the account security person verifies your identity, s/he will reset your security questions. You will then be able to go to http://appleid.apple.com sign in and enter new answers to the security questions which, of course, you will write down and save.
While you are in there, be sure to enter a 'Rescue Email Address' in the event you forget or loseyour answers, the system be able to mail them to you and you can avoid a redux of this whole exercise.

If you don't have a rescue email address (you won't be able to add one until you can answer 2 of your questions) then you won't get the reset option - you will need to contact iTunes Support or Apple to get the questions reset.
e.g. you can try contacting iTunes Support : http://www.apple.com/support/itunes/contact/ - click on Contact iTunes Store Support on the right-hand side of the page, then Account Management , and then 'Forgotten Apple ID security questions'
or try ringing Apple in your country and ask to talk to the Accounts Security Team : http://support.apple.com/kb/HE57

I need help. I cant answer any of the security questions because none of them apply to me and the ones that do i dont remember, now i cant buy anything because i need to answer these questions. Is there a way to turn them off...
I too am stumped by the security questions. I can't answer any either. As a result, I can't even download free apps.
The problem also with predetermined security questions is that they're discoverable. Say you did have a favorite teacher. You or one of your friends might have mentioned that teacher on Facebook or a blog.
Another problem with predetermined questions is that "dunno" could be an answer – an easy answer to crack!
The best is to give the user the option of creating their own security questions. -

Did you do the Super Duper clone before you tried to reinstall the OS? If so, the original install disc is likely earlier than 10.4.11, and that may be why you are getting the message.
When trying to reinstall the OS, did you select the "Archive and Install" option? If not, you might try that. Here's a link that might help:

You need to ask Apple to reset your security questions; this can be done by clicking here and picking a method, or if your country isn't listed, filling out and submitting this form.
They wouldn't be security questions if they could be bypassed without Apple verifying your identity.

Can't choose new security questions?
When I upgraded to iOS6, I got the message to create three new security questions to improve Apple ID Security. I can't download any new apps now because of this message. However, when I click on Continue to set them, it asks for my password then takes me to a browser page with three items: Choose Question and Answer for each. You can't choose a question as there is no link to do so. You can only enter an answer and when I enter three answers and choose submit, I get a session timed out response. Three times.
I haven't been able to fine anywere online to enter these questions either. On my account, there is still just one security queston and no option to add others. Anyone found a solution to this yet?

I can´t answer the security questions, because only the half site comes up. First line I see is "security question" last line is "alternate e-mail". On top I miss e-mail and Password, at the end I miss the age compared with support info.

I forgot all of the answers to my security questions and when I set up my apple id I did not add a rescue email, I recently got a new iphone and I cannot purchase anything on it because it asks me to answer my security questions which i can't reset because I do not have a rescue email. what do I do?
this does not help. I have forgotten ALL of the answers to my security questions. I can NOT reset the answers to my security questions without answering two of the three security questions. The other way to reset the answers to these questions is for apple to send me an email on my rescue email addres which I DO NOT HAVE.
