You can remove your Apple ID from a device by going to iTunes on your computer and then Sign In to view your account. You should then see and option that says Manage Devices and then you can remove it from the list.

Keep powering your phone off and back on, it can take up to 2 hours for a normal service provider to finalize the activation process. When you got your new iPhone (congrats by the way) you should have received a little bubble that appeared that told you when the phone was activated, did you see that?

Connect the old phone to your computer, right click on it in iTunes, and choose Transfer Purchases. That should put all of your music in iTunes except any tracks that were not purchased from iTunes. Depends on why it's ineligible. If it's under 96 Kpbs, try right-clicking on the file and choosing Create AAC Version. (If you don't see this option, go to iTunes>Preferences, click "Import Settings..." then choose "Import Using: AAC Encoder" and "Setting: iTunes Plus".) Then rescan your library to see if this version will add to iTunes Match. Or, after changing your import setting, just re-import the song to your iTunes library.
If the song is over 200 MB you would have to reduce it's size to make it eligible.

How do i take the playlist from itunes and put it into my iPhone 4S?

I made a playlist via itunes and went to sync it with my iPhone 4S with no issue but the playlist didn't change over. Now i could just as easily make a playlist via the iPhone 4S and be done with it but I shouldn't have to and either i put all my music into a playlist and select playlist only then it might work but i'm looking for a suggestion to fix this issue with minimal affects. If the suggestion is to just put it as a playlist via the iPhone so be it. But I feel that's a bandaid on a missing limb. i wish Apple wouldn't make such drastic changes without user input.

Everything works as it always has regarding syncing in version 11 as it did in previous versions.
Select the content desired to sync and sync.
If you want to use "drag and drop" enable the option to manually manage content on the device.

Maccbook will not power on, so there is no way I can deauthorize using that computer.

You would have to contact itunes support and ask if they will allow anther deauthorize all.

Not sure where you backup to.
ITunes keeps the latest backup only.
Icloud keeps the 3 last backup.

I would just follow the instructions from this article to give your unused 4s to your spouse. Since you have already started using your 5s, you will still be receiving iMessages on that phone and will have no need to receive them on a device that has been replaced.
