My phone number appears on my daughters iTouch and she can receive my text messages. If I need to creaye a new user account for her itouch, can i use the same iTunes account or do i need to create a new iTunes account?

Create a NEW account/ID for her using these instructions. Make sure you follow the instructions. Many do not and if you do not you will not get the None option. You must use an email address that you have not used with Apple before.
Creating an iTunes Store, App Store, iBookstore, and Mac App Store account without a credit card
Use the new ID on her iPod but only for:
Settings>Messages>Send and Receive
Settings>FaceTime
and Settings>iCloud if you want her to have separate Contacts Calendar and some other things.
Continue to use the same/common Apple ID for Settings>iTunes and App stores so you can share purchases.

Many developers when they updated their apps to support iOS 5 and 6 dropped compatibility for iOS 4.2.1 and earlier.
Your only way is if you have the version that was compatible on your computer such as in the recycle bin/trash or in a computer file backup. In that case delete the current version that is in your iTunes App library and replace it with the older version. Then sync.

When I do the same thing in IE it downloads the statement. When my husband does the same thing in Firefox 3.6.17 it downloads the statement. It worked until I upgraded.

There was a plug-in called "Adobe Acrobat NPAPI". With that disabled and the PDF Browser Plugin 2.4.4 enabled, I can now open PDFs in Firefox.

Assuming you have an iPad wi-fi+3G, were you connected to an available wi-fi network when attempting to download the application?
Regardless, installing such a large app on your iPad is probably best via the iTunes sync process which might be required with such a large app.
