I need to restart my iPad mini from scratch, it has been wiped clean and disabled on ITunes. I want to know how to restart the iPad and put a new password in , connected to iTunes and don't know where to start.....

You will need to restore your device as New to remove a Restrictions passcode. Go through the normal process to restore your device, but when you see the options to restore as New or from a backup, be sure to choose New.
You can restore from a backup if you have one from BEFORE you set the restrictions passcode.

The home screen says that it has been "disabled." When connecting to iTunes I am being told that I need a password to connect. My first iPad issued from work had some technical issues and it was sent back to Apple...as a result when I received my new one I backed everything up from iCloud, and never needed to synch it with iTunes for my use. We are fellow users here on these forums, you're not talking to iTunes Support nor Apple. I don't know where you originally posted this question, but it's only just appeared in the Using iPad forum.
If you can't remember your password then you can reset it via http://iforgot.apple.com
If you can't get it reset via that site then you will have to contact Support in your country, they should be able to reset it for you (they can also reset your security questions if you've forgotten their answers).

The instructions are in that link. Just follow them VERY CAREFULLY. Especially the part about:
Turn off your device and leave it off.
Plug in your device's USB cable to a computer with iTunes. [but DON'T connect it to the iPhone yet]
Hold down the Home button on your device as you connect the USB cable. Keep holding down the Home button until you see the Connect to iTunes screen.
When you see the iTunes screen, release the Home button. If you don't see this screen, try steps 1 through 3 again.
When your device is connected, iTunes will open. You'll see a message saying that iTunes has detected an iPhone, iPad, or iPod touch in recovery mode.
Hold down the Home button on your device as you connect the USB cable. Keep holding down the Home button until you see the Connect to iTunes screen.

If problem what happens or does not happen and when in the instructions? When you successfully get the iPod in recovery mode and connect to computer iTunes should say it found an iPod in recovery mode.

forgot_password.php, contain form and php mail function
error.php, a redirect page if the mail function doesn't work.
And then, use your Dreamweaver to make a recordset, called: rsForgotPassword

Filter: email, Form variable, =, email. Use form variable to pass the value from the form.

You'll need to connect it to the iTunes library you normally sync it with and restore it. If iTunes asks you for this passcode before it will let you proceed, connect the iPod to iTunes in recovery mode instead using the instructions in this Apple support document.

Try this - Reset the iPad by holding down on the Sleep and Home buttons at the same time for about 10-15 seconds until the Apple Logo appears - ignore the red slider - let go of the buttons.
