ID CS3 Custom Page Set-up

Question

I am trying to set up a custom page for DVD Case inserts.  I have DVD Case Inserts sheets that  have perferations for the edges and the folds down the center. I am having a terible time setting up the page.
The full sheet is 11.75 by 8.25" I set custom page size to that measurement
The perferated area is in .5 inches on all sides.  I set the margins to .5"
When I print every thig prints shifted down 5/16" and shifted to the right by 5/8'
I have the same problem on two computers and two differnet printers, so I assume it is a settigns thing.  What am I doing wrong?

I am not so sure what you mean.  I just did a test with a 8.5 by 11 page with the Use Document Bleed Setting on and off and it looks the same and nothing shifts. Doesn't bleed just spec the printed page slightly larger, like .002 inches?
Why does a custom page shift all over the palce.  I beginning to think it is a falw in the S/W.

When you open the Document Setup dialog there is a button to "show more options" which will give you fields for bleed and slug. Amounts you enter into these fields are indicated by new guides drawn outside the page area on the pasteboard, and these are the amounts that would be added around the edge of the page when you select Use Document Bleed or InClude Slug area, respectively. I asked before, and you didn't say, if you are also using any printer's marks, like crops or page information. Marks also add area around the edge of the page, and if you are using upper left positioning, turning off crop marks would cause the page image to move up and to the left on the sheet.
Stix Hart raises a good point. Desktop printers, and even most commercial copiers, have pretty poor page-to-page and front-to-back position control due to the way they feed paper (that's why you need bleed). Tolerances are generally around +/- 1/16 inch or 1.5 mm, so it isn't unusual to see total shifts from one extrem to the other of around an eighth of an inch (1/16 on either side of where things "should" be) but a 5/16 shift  is really too much to see from that effect, I think, and happens to be realitively close to the default offset for crop marks or could be the total offset if you included both a bleed and bleed marks.
Can you post screen shots of both the Document Setup dialog and the Print dialog Marks and Bleeds area, please. Use the little camera icon to embed the images into your post.
