Initialization stuck at 0%
G5 dual 2.3 Ghz Xserve running 10.4.3 brand new out of the box. Xserve RAID with 7 drives all in the upper bay brand new. I have tried creating a RAID 5 array but initialization is stuck at 0%. I tried creating a RAID 0 with two of the drives and still get 0%. I have tried with RAID now enabled and disabled. I cannot see any arrays in Disk Utility.
RAID admin says the fibre channel links are up and the link lights are on.
Fibre Channel Utility is telling me the port link is down, card is in slot #2
RAID Admin 1.5 with the latest firmware and the Apple Fibre channel card in the Xserve.
Any ideas ?
OK I reseated the Fibre Channel Card and now I get some information in the Fibre Channel Utility. But the Array doesn't appear in Disk Utility ( I am formatting a 2.73TB RAID 5 with RAID now option). The system profiler does show there is communication with the XRaid.
Any ideas why the array doesn't show up in Disk Utility ?

LMS 4.2.3 - DCRServer stuck in state: Waiting to initialize

One of my installation has gone stuck ... OVA install with SP3, has run great until now ...
I had problems with DCR, and restarted the daemons via Shell-CLI.
Did not start up again ...
Then reload via nomal-CLI
Did not start up again ...

... This happens when there are lot of files in < NMSROOT>/log directory.
So i have removed all files i /var/adm/CSCOpx/log/ ( i only had 252 files, which is not "a lot")
started again:
/etc/init.d/dmgtd start
But still same issue ...
I am think of upgrading to SP4, but the bug is still open in SP4 aswell, so i am not sure this will fix it

I think the last file from your explanation was destroyed by ptt

Stuck on Processor Initialization
Hi all,
Tonight I decided to upgrade to Windows 7. I preordered almost 2 months ago so I was quite eager to install it :D I also purchased 2 more gigs of ram.
So after I backed up PC I turned of PC, grounded myself and installed the extra 2 gigs of ram. Cleared CMOS, booted PC and it went into Windows XP no worries. Shut down PC again, un-plugged my IDE HardDrives and all USB devices. Installed Windows 7 and all went ok, had a play around with W7 after about 30 mins I shut PC down again, grounded myself, plugged in IDE HardDrives. Switched on PC and it made a 3 second beep and repeated every 5 seconds or so. According to D-Bracket it stuck on "Memory Detection Test". So I took out the modules one by until PC started to boot. Replaced modules and it passed the memory test but then nothing else happened ie not posting and gets stuck at "Processor Initialization". I know this because the d-bracket lights are as follows 1-Red, 2-Green, 3 - Green, 4 - Red.
There was at one point where all 4 lights on the D-Bracket were red and nothing else happened. Discovered there could be an issue with the processor. Any way I Cleared CMOS and again stuck on "Processor Initialization". I'm starting to feel that somehow I fried the processor, what you guys/gals think?
any help appreciated.

You reminded me, I forgot to say I did accidently remove the power to the fan for the main board but I did put it back straight away!
Main Board power cable connected defo.
This is what I did just now:
1. Removed all ram.
2. Rebooted and got the beep.
3. Tried all memory sticks in slot one and each time D-Bracket reported "Processor Initialization"
4. Replaced all Ram.
5. Removed SATA HD, USB PCI Card and Graphics Card turned on PC and still getting "Processor Initialization"
So as it stands there is nothing connected apart from fans and the system wont get past "Processor Initialization". -
Neo2 plat stuck at processor initialization
help,out of the blue, my k8n neo2 platinum won't boot. the diagnostic bracket is showing red,green,green,red which the book says is processor initilization. i swapped in a known working processor with no luck, swapped known working memory, jumpered the cmos reset, removed the battery, disconected the drives. nothing has worked. any ideas? the motherboard just go bad? it's been bullet proof up 'till now. any help would be appreciated.

Cannot restore iPod Touch 2G - stuck on "operation 28" case #108375671
iPod Touch 2G with 8GB, got it from Apple Store last week.
I tried restore on 3 different computers - 2 laptops and 1 desktop, running XP SP2 and Vista. Resintalled iTunes(latest version) and Apple Mobile Device drivers a few times. Also created new accounts to restore. None of these works.
Most of the time iTune stuck on the "Waiting for iPod..." screen. Checked the iPodUpdater.log and found the restore process stuck on "operation 28 progress -1" forever. Operations 4, 42, 5, 6, 7, 8, 9, 3, and then 4 seems to be successful. iTune was also able to read the SerialNubmer and Hardware model.

Looks like this one passed "Operation 28" but failed on "Operation 11".
There is always an error message on the file "device_map.txt" - which it does not exists
Contacted Apple Care and talked to 2nd line support. 2nd line support said they don't know how to interpret the log file. The log file can only be interpreted by the engineer.

Unable to initialize hdv deck problem
Hi all,
I'm having the 'unable to initialize hdv deck' problem. I am trying to capture HD footage from a Sony HVR-A1E camera. iLink and down conversion are off (like they should be) and I have just captured a HD tape before the problem started.
I don't understand how I could capture HD footage just fine and then without touching any settings (just trying to start capture again) I suddenly get the error message, "Unable to initialize HDV deck. Please make sure a deck is connected and try again."
Any ideas?Definitely. Shot on HDV, definite difference between the clips I've captured so far (HD capture that worked fine before this is high quality sharp image, where as when capturing in SD, image quality is poor and smaller).
Everything has been filmed on the same camera with no setting changes between shots. Like I said before, I have already been able to capture today in HD just fine, but for some unknown reason (which I don't think is the prefs.) capture will not work. I can 'trick' FCE into HD capture but it gets stuck on cueing tape (even though video is playing fine on camera). -

I had been trying to convert a physical standby to logical standby database. Both the primary and standby reside on the same machine.
The physical standby was created with a hot backup of primary.
I had been following document id 278371.1 to convert the physical to logical standby and used the following steps:
Relevant init parameters on primary:

8) Verified that the supplemental logging is on.
9) ALTER SYSTEM ARCHIVE LOG CURRENT;
10) Checked for the checkpoint change no. at this point which is 72403818 and is present in archive log file 1_62_775102253.dbf
11) EXECUTE DBMS_LOGSTDBY.BUILD;
12) ALTER SYSTEM ARCHIVE LOG CURRENT;
13) Checked for the archive log containing dictionary build which is 1_64_775102253.dbf
14) ALTER SYSTEM DISABLE RESTRICTED SESSION;

I also greped for lsp and lcr processes and found that lsp is up but do not see any lcr.
The logs are getting transported to the archive destination on standby whenever they are archived on primary but are not getting applied to standby.
Also in case the standby is down while a log is generated on primary it is not automatically transported to standby once the standby is up, means gap resolution is also not working.
I see the following in alert log every time I try to restart the log apply, everything seems to be stuck at initialization.

You have db_name='test' on the Standby, it should be 'asadmin'
fal_server=test is set like this on the standby, it should be 'asadmin'

Tecra M3 - Stucks at the Toshiba screen
Hello everyone,
I am trying to use the Product recovery disk on my Toshiba Tecra M3, however it does not seem to do anything.
I have inserted the disk, turned off computer > started computer again pressing F12> i get the enter HDD password which i do sucessfully> after which it seem to get stuck a a screen with TOSHIBA and all devices icons. after which it does not do anything....
does anyone have any idea on what to do next? or how to use the product recovery disk?

In this case you can contact your authorized service provider. Maybe you can have a new one from them. Just ask for that! ;)
Alternative you can use a Microsoft Windows disk. All drivers and tools can be downloaded on the Toshiba website. -
Creating a "Business Service" from a WSDL: Can't initialize WsdlEntryHelper
I'm working through a "HelloWorld" setup of Weblogic (10.0.3.5) and OSB (11.1.1.5.0) using OEPE 11.1.1.7.2.
I have a OSB Project with a Weblogic Web Service. I've generated a WSDL and when I try to copy/paste or drag-and-drop or load the WSDL file through the Business Service Configuration I always get the following error:

W510 Solid State Drive (SSD) Gets Stuck Causing BSoD. Why?
Hey guys, this has been a problem for a while now, and it is really mind boggling ... I have a Lenovo ThinkPad W510 laptop with a 256GB Crucial M4 SSD drive. Ever so randomly the drive just hangs, the LED light that the drive is trying to load something stays "ON" for up to 5 minutes or so before it finally goes into a Blue Screen of Death (BSoD). Once it goes into BSoD the laptop reboots and it gets stuck on the ThinkPad motherboard logo, eventually it will display "ERROR 2100: HDD0 (Hard Disk Drive) initialization error (X)" Where X is usually 1 or 2, sometimes 3 but rarely. It will not function again until I physically turn it off and back on using the power button.
A little bit of background about the problem, this is my 3rd Crucial 256GB M4 SSD, Crucial has been very helpful and they went as far as replacing the drive twice, i am starting to highly doubt its a drive issue, I think it could be something from the laptop itself. To be more clear, my laptop has also been serviced back in March (Motherboard was swapped as well as keyboard), parts failure. It was all done in very timely manner and everything was OK after the swap.
A representative from Crucial told me that this is a classical problem with the SATA controller driver handling TRIM, he said the drive is getting stuck in an infinite TRIM state which causes the BSoD, after that the drive goes into some sort of a "panic" mode to protect itself and doesn't function again until a full power cycle. (thus the need to turn off and then on the laptop) he also added it could be fixed by installing the correct SATA controller driver.
I have tried to use the default windows SATA controller driver, and I have tried to use the Intel one too, and it seems to happen less often with the windows drivers, but overall, it happens at least once a day. It is really frustrating and I am not able to get anywhere with it.
Any help or advice is highly appreciated. I am considering getting rid of the Crucial SSD altogether and just using Intel SSD instead, but I am really doubtful that it is the SSD's fault especially that its the 3rd one I use. I also know a couple of friends who own ThinkPads with Crucial M4 SSD inside them, and they seem to be working very well for them.

Ok so I have applied the fix you have mentioned above, and it did improve (lower) the frequency of the problem happening. It really improved a lot significantly when I uninstalled the Intel drivers and used the Windows 7 built in SATA controller drivers.
However, ... it still happens ... less frequent, but it still takes place, my SSD still gets stuck at least once in 3 days now, or once in 2 days (after severe testing) and the same scenario happens. I am really stuck at this point, and not sure which direction to take.
Any help would really be appreciated, if you have / know any method to debug the BSoD's I get and a way to locate the source of the problem I am willing to go through it 1 by 1 and solve this problem before deciding to finally just giving up on the drive altogether.

K9N2 SLI Platinum video won't initialize with monitor connected
Hi,
I've gone through the RMA process with my vendor and I am on my third motherboard right now and I am still having an issue (so, basically, nightmare scenario here ). The first and second motherboard failures seemed slightly different but the failure seems to be the same for the second and third and it is making me think that another factor may be at play other than the motherboard. All of the failures have been with the integrated video (which is all I plan to use at the moment --I don't even own a PCI-E video card right now nor have any plans to get one in the near future).
In the first motherboard, everything seemed to work (as far as I could tell --4 green diagnostic LED's and all that) but no video signal was produced. Testing with an antediluvian Virge DX based PCI card (some of you kiddies may be too young to remember such things) did show normal booting with that video card (as far as I allowed it to go) and I suspect, from the pattern of hard drive activity, that with the integrated video the boot process was also normal (I just couldn't see it because of no video signal being produced). But that is past and has been RMA'd.
The replacement motherboard and the motherboard replacing the replacement motherboard, on the other hand, have seemed unable to initialize video whenever my monitor is connected but if the monitor is not connected, they work fine (but then video defaults to a 1028x768 mode and I cannot run my monitor at native resolution). Curiously enough, my second motherboard worked correctly twice before exhibiting the described behavior making me think, at the time, that I just got really unlucky and simply had had it fail shortly after testing it those two initial times.
Not being able to initialize video while the monitor is plugged in means that if I try to start it while the monitor is attached, the motherboard will not even POST and that the diagnostic LED's will be stuck at the video initialization stage (three reds, and one green at group 4).
Furthermore, if I start it with no monitor attached and plug the monitor in after I get 4 green LED's, it will start booting the OS (Windows XP) but the screen will go black when the Windows XP logo should be appearing. I can also leave it unplugged until it should be at the Windows XP logo stage (or plug it after the 4 green LED's and unplug it again before the video mode switch happens) and when I plug it back in I can see the Windows XP logo. However, as soon as it switches to the Windows XP desktop, I get a black screen.
And again, if I leave the monitor unplugged until the Desktop should be showing up and I plug it in at that point (or go through the process of plugging and unplugging the monitor while avoiding having video mode switches happen with the monitor plugged in) I do get a Windows desktop at the 1024x768 resolution. Once in the desktop, as far as I can tell, I can do anything there that I normally would be able to do (so I mostly have a working system, as far as I can tell). I have even installed drivers, verified that the sound works, etc.. I will still do a repair install at some point because I am not doing this the "right way" and I probably needed to do it anyway (though I think it's using the right kernel and everything --showing 3 CPU's, etc.) but I am not looking for answers with that as my issue shows up before the system even goes through POST. Again, note that I cannot even take advantage of my monitor's native resolution with the current kludge.
Curiously enough, if I try to switch to my native resolution of 1920x1200 it will do it but only by switching me to a virtual 1920x1200 desktop on a 1024x768 display (so there's no actual video mode switch involved and the desktop becomes bigger than what my monitor can show). It's as if part of the subsystem is reading the monitor and "knows" that it supports 1920x1200 but another part of the subsystem is just assuming some sort of 1024x768 default common lowest denominator for digital displays. I attribute this to the fact that video has been initialized with no monitor connected.
I originally had a 350W Enermax power supply (which worked as long as the monitor was not plugged in) supplying 32A on the +3.3V rail, 32A on the +5V rail and 26A on the +12V rail but I replaced it with the power supply shown on my profile (which was probably totally unnecessary as I'm only using the integrated video --but hey, I now have a blue light coming from my power supply so that makes it all worth it ). I suppose both power supplies could work badly with this motherboard model but it seems exceedingly unlikely that that would be the case (specially since everything works as long as there is no monitor plugged in during video mode switches).
I currently am running it, on cardboard, with a single stick of RAM (on the slot next to the CPU) and nothing else connected to it except the power supply (including JPW3 & JPW4), the CPU, a hard disk with a working Windows XP install, a mouse on the mouse port and a keyboard on the keyboard port (specification as on my profile minus one RAM module). I did play with RAM timings at the request of tech support but I can't imagine how it could be a RAM problem when everything works as long as a monitor isn't plugged in during video mode switches.
I did update the BIOS from 3.4 to 3.5. 3.5 did not seem to be available at the time of my previous motherboard and I knew it was a real long shot for this to be a BIOS issue that would be fixed with the 3.5 update (specially since the update mentions only CPU support stuff and nothing about video) but I am desperate. Yes, I have done the whole resetting the CMOS thing.
I am even considering the possibility that there may be some weird incompatibility between the video on this board and my monitor (under the assumption that, quoting Holmes, once you have eliminated the impossible, whatever remains, however improbable, must be the truth). I don't even know how that would work. Note that my monitor worked in my previous system with the digital input (my previous system is no longer available to me for testing --but I did get a chance to test it, after having problems with my first K9N2 SLI Platinum motherboard which I returned, and it worked). Note that my monitor works with the system in question (I simply cannot have it connected while there's a video mode switch under way). Note that the lack of a picture seems to be related to the motherboard as evidenced by it being stuck at the video initialization stage if I attempt to start it with the monitor connected.

Addendum: As an additional note, I have tried the other DIMM on the same socket by the CPU with the same results (everything works as long as there's no monitor connected during a video mode switch). I also managed to make a bootable Memtest86+ (is that what people use to test memory these days?) USB stick and I am running it right now. It has shown no errors right so far but it has only been running a few minutes. I do not really expect any memory errors to be at fault since they would have to be magical memory errors which only cause problems when there is a monitor connected while a video mode switch is happening but I'm desperate to try anything to find out what the issue is. -
[Solved] Failed to initialize the NVIDIA kernel module

I've just made the jump to arch and have a fresh install. I'm having trouble getting the nvidia drivers to work. I am currently using the nouveau drivers without issue.
The method I am using - after much trial - is:
- uninstall nouveau and mesa lib
- install nvidia package
- remove nouveau module entry in mkinitcpio.conf
- run `mkinitcpio -p linux`
- edit .conf in xorg.conf.d with minimal required settings. Currently:

The only entry I found in `dmesg | grep nvidia` related to nvidia-drm, and seemed to say it was loaded successfully. I didn't save the exact entry and i've reinstalled nouveau now so that I could post this.
Any help much appriciated. I've been at this for a while. Learning a lot in the process!
Last edited by gnibbler (2014-01-04 22:31:49)Success! Several days of trying various things and I finally give in and make a post to this forum. One more attempt - which was only really so that I could add modprobe info to this post - and it works.
I had at some stage installed the driver xf86-video-nv, and this may have been causing the clash. I un-installed it, and followed the procedure above to reinstall nvidia.
I think the main hang up I'd had is that I had not been rebuilding my kernel image with mkinitcpio after installing nvidia during my previous attempts.
I hope some info in here helps some other stuck archer.
Edit: Thanks graysky. I had found the right nvidia driver using the arch wiki and followed this advice to remove the nouveau hook. Much appriciated.

Gnome stuck in fallback mode - not recognizing nvidia drivers
Every time I start a gnome session it is stuck in fallback mode. Checking under System Settings > Details > Graphics I see that the Driver is listed as Unknown and Experience is Fallback. Forced Fallback Mode is OFF. I cannot get it to recognize the nvidia driver (I have also tried nouveau but it did not recognize that either). I have tried re-installing the nvidia driver and rebooting, but I can't seem to fix the problem . Here are the logs that I thought might be relevant. 3779There is one thing I forgot to clarify, have setup up the pc as a server so I do not have direct access to the server. Everything is forwarded over ssh (using nx to connect to the server and start a gnome session). Starting KiTTY (a modification of PuTTY) with x11 forwarding I do not see any windows when I run "xinit -- /usr/bin/X -nolisten tcp". I get the following:

I remember when I would start the pc that startx I could never get to run, I would always start the gui using gdm, which was the only thing I could get to work. I do not have a .xinitrc. I believe the nx connects and issues gnome-session to start the session but I am not sure.
