IPhone 6 asks for a passcode but there is no passcode on the phone

About three weeks ago I got an iPhone 6 and have had problems ever since. Initially I put on a passcode but then removed it because I didn't want to keep clicking all day long (have repetitive strain). But the phone kept asking for the code. So after numerous calls to Rogers and Apple support, I went through various resets, deletes, restores to factory settings, restores with and without backup, restores without syncing the phone to iTunes, etc.. Same problem. Took phone to Apple and they replaced it. That worked for about four hours – new phone never had a passcode but keeps asking me for one, primarily when I use Hey Siri, but new phone couldn't have a memory from my old phone; it was never connected to iTunes.
Has anyone had a similar problem? Is this trending with Hey Siri? I am not locked out of my phone. And neither phone, the first one or the new one, remembers the original passcode. I can put in the original passcode or another passcode, or I can simply hit the accessibility device that I use and click "home screen" and bypass the problem altogether. The actual message that I get is I'll say, "text my sister" and Siri will say, "you have to unlock your iPhone." Then I get a passcode prompt, not for the Apple Store but as though I had a real passcode.
Any suggestions? Wondering if other people have seen this and solved it. I have read about putting the phone in recovery mode. I'm thinking about doing that but the whole thought of re-entering my data for the 12th time – killing my hand – and then retraining Siri is discouraging. She doesn't know anything at all in the beginning. I restored her from my iPhone 4S, so she works very well in terms of knowing me and my vocabulary now. Very happy with the phone except for incessant passcode prompt.

Make an appointment at the genius bar of your local Apple store or contact AppleCare. Some Apple Stores in the US can replace the display in an iPhone 5 for $149. If it requires more than that, a replacement phone will cost you $229.

    That's because the device has not been synced to that computer.  As per my earlier instructions... you will have to force it into recovery mode & restore as a new device.

    If that's the case you may have to disable wifi syncing in her user account using her iTuens library.  I'm really not sure though.  If someone does, hopefully they'll jump in here.  The problem with chaning libraries is that when you do, iTunes will delete any iTunes media on the phone and replace it with the media in the iTunes library you sync with so hopefully this won't be necessary.

    I can not get to iPhone screen, so no way to get to Settings. That is the problem. When I connect phone to iTunes I get screen asking to insert SIN card. On the phone I'm getting Cinfigure screen asking Launguage , Country or Region, Wi-Fi, ( I can connect to my home wi-fi), and last screen "Activating your iPhone". After two minutes of waiting IM getting message: Your iPhone could not be activated because the activation server in temporary unavailable. try connecting to iTunes or try later or contat apple.com/support.

    Forgotten Restrictions Passcode Help
    You will need to restore your device as New to remove a Restrictions passcode. Go through the normal process to restore your device, but when you see the options to restore as New or from a backup, be sure to choose New.

    Thank you. I told it to the apple store in Southampton and they were fab. I was lucky and there was no water damage, the water indicators didn't change colour. It was just bad luck my screen failed at the same time as it got wet!! Screen changed and all is well. Thank you Genuis Tom and West Quay!!

    Contact iTunes Customer support... it is not possible to bypass Activation Lock.

    ‘iPad is disabled’ fix without resetting using iTunes
    Today I met my match with an iPad that had a passcode entered too many times, resulting in it displaying the message ‘iPad is disabled – Connect to iTunes’. This was a student iPad and since they use Notability for most of their work there was a chance that her files were not all backed up to the cloud. I really wanted to just re-activate the iPad instead of totally resetting it back to our default image.
    I reached out to my PLN on Twitter and had some help from a few people through retweets and a couple of clarification tweets. I love that so many are willing to help out so quickly. Through this I also learned that I look like Lt. Riker from Star Trek (thanks @FillineMachine).
    Through some trial and error (and a little sheer luck), I was able to reactivate the iPad without loosing any data. Note, this will only work on the computer it last synced with. Here’s how:
    1. Configurator is useless in reactivating a locked iPad. You will only be able to completely reformat the iPad using Configurator. If that’s ok with you, go for it – otherwise don’t waste your time trying to figure it out.
    2. Open iTunes with the iPad disconnected.
    3. Connect the iPad to the computer and wait for it to show up in the devices section in iTunes.
    4. Click on the iPad name when it appears and you will be given the option to restore a backup or setup as a new iPad (since it is locked).
    5. Click ‘Setup as new iPad’ and then click restore.
    6. The iPad will start backing up before it does the full restore and sync. CANCEL THE BACKUP IMMEDIATELY. You do this by clicking the small x in the status window in iTunes.
    7. When the backup cancels, it immediately starts syncing – cancel this as well using the same small x in the iTunes status window.
    8. The first stage in the restore process unlocks the iPad, you are basically just canceling out the restore process as soon as it reactivates the iPad.
    If done correctly, you will experience no data loss and the result will be a reactivated iPad. I have now tried this with about 5 iPads that were locked identically by students and each time it worked like a charm.

    No that is not possible, if you had a passcode you would have had to input the passcode on the screen when you choose to erase the iPhone, given that you erased it in the settings menu. If you've erased it in iTunes it could be that Find My iPhone was still active and then the buyer needs your Apple ID credentials.
    A way to circumvent this issue is to login on www.icloud.com and erase/delete your iPhone from Find My iPhone. That way Find My iPhone will be disabled on that iPhone and the buyer can activate it during the initial setup.

    I started to notice that my sleep button started to not respond like normal. I would have to push it more and more times for my phone to turn off. Only the left part of the sleep button works.     As I mentioned, someone else's personal experience may not be duplicated. It all boils down to what the particular Genius has to say at the date and time that you show up. The worse they can say is they will not replace the phone under warranty and you would be offered an OOW replacement. If you have Apple Care +, it would be a $50 deductible, where the OOW replacement cost for the iPhone 5 in the US is $229.
