IPhone appears not unlocked, but direct from Apple?

Slightly confused by the following error on my 4S.
I obtained this phone from Apple, Cambridge 1.5 years ago. As such, I was told it was an unlocked model and would run on any UK network.
Since factory wiping it and installing an O2 Sim card, I am getting the following error:
The SIM card that you currently have installed in this iPhone is from a carrier that is not supported under the activation policy that is currently assigned by the activation server. This is not a hardware issue with the iPhone. Please insert another SIM
I have always used a sim only sim card, so I have no provider to call as such, as they have no responsibility over the phone. Must I go back to my Apple store and get them to sort it out, have never had this problem before?

Thanks for your prompt message. I do apologise, I had some trouble in posting this question, the website kept saying I was not allowed to post and I didn't quite put in as much detail as I would have liked.
Up until last week, I was using an Orange/EE sim card in the phone, without any errors or problems. I had used this sim in the phone since I got it from Apple. I now have a new O2 Sim card. Before removing the Orange card, I factory wiped my 4S and then inserted my new O2 one. That is when the message came up.
The phone is fully updated to the latest iOS.

    Only AT&T is authorized to unlock the phone but you will have to meet their requirements, generally you would need to have service with them prior to them agreeing to unlock your phone.
    Return the phone if you can and see if the Apple store is selling any unlock ones.
    As of right now you won't be able to use the phone unless you activate it thru AT&T.

    If you bought it directly from Apple as unlocked it should be unlocked for all networks. There are phones sold that WILL lock to the first carrier it is used with, but this does not apply to phones where the receipt says "unlocked".
    I have not had good experiences with Carphone Warehouse.

    My daughter ordered an iPhone (5s, 32gb, black) directly from Apple on the first day possible. She still has not received it. I ordered one from ATT and rceived it in two days. Does anyone know whether this wait from Apple is normal?

    She should call the Apple Store ask ask about status. With the iPhone 5S now showing 2-3 week delivery, I would think it unusual and an indication of a problem with the order that a 5S ordered the first day hasn't yet shipped.

    It won't recognise it at all, but luckily I backed up very recently.
    I've booked an appointment with an Apple store who promise a working iPhone within 10 minutes of your appointment, whether fixed or a brand new iPhone.

    If you are running Windows Vista or later, open up your Start Menu and search for "Services."
    Click on the icon that looks like a smal gear.
    Once you have that window open, Look for the service that says "Apple Mobile Device"
    Once you click on it once, to your left of the window, click "Start this service"
    After you have done that, with the same service still highlighted, Right click on it then click "Properties"
    Once that menu is open, Find the drop down box and make sure the start up is set to "Automatic"
    After these steps are completed, you should have no problem with your iPod Touch, or iPhone not showing up in the iTunes window.

    I bought a cellular iPad Air 2 directly from Apple. I would like to (also) use it with Verizon. I went to a Verizon store and asked if I could get a Verizon SIM card, but the they claimed that is not possible. You can only use the iPad Air 2 on Verizon if you buy directly from Verizon. Is that true? I asked in the Apple store, but did not get a clear answer.

    You can activate the iPad Air 2 or iPad Mini 3 on any network, you just need the proper SIM card, and you have the freedom to change carriers, but will have to change SIM cards.  For some reason, the people in the Apple store are not well informed about the interchangeability of the new iPads. There are two kinds of SIMs now which cover all 4 current carriers: the Apple SIM, which can be used on AT&T, Sprint & T-Mobile, and the Verizon which is exclusively for Verizon.
    I read up about the features of the new iPads, and the interchangeability of the new iPads is the feature that convinced me to go ahead and spurge on a new one.  I bought an "Open Box Buy" iPad Mini 3, and when i trieed to activiatee the cellular, I found that it had an AT&T branded SIM, and I got an "Invalid SIM" message.  I made an appointment at the Genius Bar at teh Apple Store, and explained that I wanted to activate on Verison and needed a Verison SIM.  The guy who was helping me said he didn't even know you could do that.  He went in the back, talked to someone to verify that I could switch carrieers.  They gave me the Verion SIM, I activated it and all is well.  Read the articles linked below, and stand yuor ground since Apple isnt giving their emplyees the latest info.  My theory is that Apple isn't making the interchangeability well known because they don't want to aileante their carriers.  (I'm sure Apple willl save a lot of money not having to keep inventory of iPads for each of their carriers, and instead only have to keep 2 different types of SIM cards on hand instead).

    If nothing shows on the purchase history on your account, and you only have the one iTunes account that the card has been linked to and it's not a temporary store holding charge (you haven't changed your card details on your account ?), then you can contact iTunes support via this page : http://www.apple.com/support/itunes/contact/ - click on Contact iTunes Store Support on the right-hand side of the page, then Purchases, Billing & Redemption.
    But if nothing shows on your purchase history then you should also consider contacting your card issuer.

    -If you established a rescue email address, there will be a link on the "Passwords & Security" page of id.apple.com.  Clicking the link will send the reset to your rescue email address (NOTE:  This is not the same address as your Apple ID email)
    -If there is no link on the page, then you didn't establish a rescue email address.  Contact AppleCare at 800.694.7466 (If you are in the US), and ask for account security.  You will need to answer some questions to verify your identity, AND you will need access to a computer to generate a temporary support pin.

    Welcome to the Apple Community.
    The Apple TV will delete content if it is used to plat something else or is turned off. It only has a limited amount of volatile memory.
    If you want to download something for watching at a later time, it would be best doing it from iTunes on your mac/PC and streaming it to the Apple TV when you are ready to watch it.

    to ask Apple to remove the sim lock directly? I know Apple in contract with providers, but an example:
    I get an iPhone from US (as an european person). It locked for one of US providers. I can activate it (if I have original
    provider's SIM or on CDMA network), but nothing else. So I get an iPod (in good case). To use this device, I must go
    to US, stay there for many months, make a contract with the provider, and after many waiting, they may unlock it for
    other fees (or not). As anyone can see, this is not a working way.
    So if the phone is clear (were on good standing on original provider, not reported as stolen device etc.) is it possible
    to request unlock directly from Apple (for fee) ?
    I know there's unofficial methods to unlock the iPhone, but everyone sleep better if everything legal.

    Yes, no one put a gun to my head, and as you can see, that's a theoretical question. In EU, Apple products
    are overpiced, and no reason to buy them. If they will provide acceptable prices for simple people, I will the

    ding, it cost some $ only. If you pay more, but not a customer, they not unlock it. Of couse, they can check
    the device history, and refuse the request, but they fully refuse any request without contract (I talk about simple
    contract, not fixed 1 or 2 years one).
