Items not showing up in CONTEXT SYNC ON COMMIT index

Question

My problem is, sometimes users can't find an item when searching (index is CONTEXT SYNC ON COMMIT), however, when I change the index column, the item shows up. I can even tell the index column was changed before at one point because i put a string representing the date and time in there.
Here's how I have it set up (very similar to this anyway):

     PARAMETERS ('DATASTORE location_appt_store LEXER location_appt_lexer SYNC (ON COMMIT)');And, of course there is a procedure location_appt_filter defined in a package search_pkg that I won't list here.
As far as I know indexing is supposed to be automatic on commit. I don't understand why any row would not get indexed. Any ideas?

The problem just fixed itself. I got irritated and quit messing with it. When I came back an hour later, it was fixed.
