ITunes deletes genius data on startup

Question

I'm runnung Itunes 11.1.2 on Windows Vista and syncing with an iPod, iPad, and iPhone.  Each time I start iTunes its like it was the first time - agree to the licence agreement, watch the video, but worst of all, it deletes all Genius data and turns off iTunes Match.  I then have to login in to the iTunes Store and turm Match and Genius back on and wait while it re-adds the same computer each and every time. With 720 albums and over 9000 songs this can take awhile.

Well, excluding those three folders didn't help.  Diagnostics shows all tests passed.  Here's at least some of the output:
