Itunes will not forget old artwork

About 2 years ago I manually added artwork for all of my tracks. If I could not find artwork I added a uniform 'blank record' picture to take the place of the musical note which itunes placed in the absence of artwork.
Now I have come to update these old pieces of artwork - but itunes will not let them go...
I have tried deleting the old artwork, and adding in the new artwork. Itunes instantly displays the OLD artwork as soon as I drag the new artwork in, even though I have just deleted the old artwork right before..
I have tried every possible combination of removing / adding, then adding in an intermediary step of changing album or artist title to try and get itunes to 'lose' the link to the old artwork which it is obviously still holding on to, but nothing works. I just cannot get itunes to forget the old artwork!
Another weird thing - in the drag and drop panel in the bottom left, the new artwork will be displayed just as I have dragged in, but the OLD artwork is displayed in the cover flow or grid views, and on my ipod.
Are there any suggestions for ridding my library of these old pieces of artwork?

Thanks for the suggestions - I'm going to try deleting the artwork folder tonight to see what success that brings. Would I delete the entire artwork folder or just the cache folder from inside that?
All of the tracks did have full tags, although the vast majority of them are single, stand alone tracks, not part of albums. I have a feeling that permissions could be an issue, as I have been having trouble with itunes organizing my files / folders in the finder as well. If I change the album name on one of these tracks for example, itunes will create the new album name within the artist folder, but will not move the file into this new folder as I imagine should be the case. It seems as if itunes cannot really get to these files for whatever reason...
I did find some sort of solution late last night. I remove the offending track from my library, selecting 'keep files'. Then select add to library and bring it straight back in. I can then alter the artwork with the changes taking place properly. Although, I did test the other issue by changing the artist / album name on one of these re-added tracks but itunes can still not seem to move them around within the itunes music folder to file them correctly. This isnt a big deal in itself but I wonder if it points to a larger problem?

    Empty/corrupt library after upgrade/crash
    Hopefully it's not been too long since you last upgraded iTunes, in fact if you get an empty/incomplete library immediately after upgrading then with the following steps you shouldn't lose a thing or need to do any further housekeeping. In the Previous iTunes Libraries folder should be a number of dated iTunes Library files. Take the most recent of these and copy it into the iTunes folder. Rename iTunes Library.itl as iTunes Library (Corrupt).itl and then rename the restored file as iTunes Library.itl. Start iTunes. Should all be good, bar any recent additions to or deletions from your library.
    See iTunes Folder Watch for a tool to catch up with any changes since the backup file was created.
    When you get it all working make a backup!
    Should you be in the unfortunate position where you are no longer able to access your original library, or a backup of it, then see Recover your iTunes library from your iPod or iOS device.

    Thank you for providing so much information about what is going on.  It sounds like you have setup iTunes to save new content to your new external hard drive in iTunes>Preferences, but it sounds like iTunes is not opening iTunes Library.itl file on the new external drive.  You have one last step to take to have iTunes point to your new external hard drive rather than your old one:
    Steps to create or choose a different iTunes Library file
    If iTunes is running, quit iTunes.
    If you are using Windows, hold down the Shift key and from the Start menu and choose All Programs > iTunes > iTunes.
    You should see the following screen:
    Additional information
    If you do not see the screen above, then you did not hold the correct key at the correct time. You may want to try again. Note that if you pinned iTunes to the Start menu programs, and open it that way, it may not work.
    Once you select "Choose Library," you will need to navigate to the iTunes Library.itl file on your new external drive.

Songs that I have purchased from iTunes after updating my iPhone sync their album artwork as well. The only songs that will not sync their artwork to my iPhone are the songs that I've purchased from iTunes before updating to iOS 5. Any suggestions?

    You have to turn your iPhone on otherwise it wont work.

    I tried to connect either of my old accounts for my iPhone 4, but iTunes will not let me in.  When I ask for password recovery no email is sent to either account, but when I try to use either email address I get an error saying that both are still registered.  THEY CANT HAVE IT BOTH WAYS!  I need to use either of these email for my current account.  How do I delete these accounts??  There is no help for iTunes online, just product support they want you to pay for!

    If You Are Locked Out Or Have Forgotten Your Passcode or Just Need to Restore Your Device
      1. iTunes 10 for Mac- Update and restore software on iPod, iPhone, or iPad
      2. iPhone, iPad, iPod touch: Wrong passcode results in red disabled screen
      3. iOS- Understanding passcodes
         If you have forgotten your Restrictions code, then follow the instructions
         below but DO NOT restore any previous backup. If you do then you will
         simply be restoring the old Restrictions code you have forgotten. This
         same warning applies if you need to restore a clean system.
    A Complete Guide to Restore or Recover Your iDevice (if You Forget Your Passcode)
    If you need to restore your device or ff you cannot remember the passcode, then you will need to restore your device using the computer with which you last synced it. This allows you to reset your passcode and re-sync the data from the device (or restore from a backup).
