You should still be able to manually manage the music on your iPhone within iTunes. Make sure that when your iPhone is plugged in, look in the Summary tab to see if “Manually manage music and videos” is enabled and you can even you turn off Automatically Sync when this iPhone is connected. Take a look at the first article below to give more information on that. As far as your other questions about calling someone, I have provided two options for you. If you buy an iPhone from the US with AT&T you will be tied into a contract with them and you need this contract to be able to activate the phone. AT&T will then unlock the phone for you, but you need to be a current customer of AT&T for them to do this for you and you need to have had the contract with them for at least 3 months before they will unlock the phone.
The only way that you can get the phone unlocked is to be a current customer of AT&T.
Again, there are warranty issues - it is only valid in the US if you buy it in the US, so any problems and you'll have to fly back there and there will also be 4G LTE incompatibilities.
Unless you want problems and to be tied into a contract with AT&T for at least the next 3 months, then don't buy it and just wait.

Make sure it's actually unlocked (unlocked iPhones are available in the US only from Apple's own stores), not just without contract. Many people don't correctly ask for and confirm that they are getting an unlocked iPhone, so be very clear about what you ask for.

You can use your iPhone 4 in Malaysia with a Japanese sim card, but you'll need to enable the data roaming/international feature with your phone comapny/carrier in Japan to be able to use the phone.
Your other option is to get your phone unlocked by your phone comapny/carrier so that you can then insert local sim cards when you travel.
The only ones who can unlock your phone is your phone comapny/carrier in Japan so you need to contact them directly to ask them if they offer unlocking - if not then you are out of luck and have to use the data roaming method.

Read your manual (you have a bookmark to it in Safari on your iPhone or put iBooks on your iPhone and download the manual from the iBookStore) and it will show you how to remove the microSIM from your iPhone. AT&T is not tourist friendly and does not offer a pay as you go plan for visitors with an iPhone. If you are going to be here for a short time you can probably buy a pay as you go microSIM and use it before AT&T notices, if it is going to be for a long time that won't work.
There is a 3rd party company that will sell you a microSIM that will work with AT&T but they are pretty pricey.
Finally, you can get a pay as you go microSIM from T-Mobile to work with your iPhone but you will only have EDGE data speeds.

Settings > iTunes & App Stores > tap your Apple ID: > View Apple ID > Country/Region
If you find that it is set to the correct region, log out of your iTunes Store account, restart your iPhone, and sign into your account again.

Since Italy has not announced availability of the new iPhones Apple has also not announced which model will work on 4G
Whilst the Singapore model will probably work on 3G there is no confirmation of which model will work in Italy on 4G
A clue may lie in the model available in other EU countries
However you will have no warranty on an iPhone purchased in Singapore in Italy .The warranty is NOT international and nor is post warranty support if you plan to bring to Italy permanently

And they were definitely purchased from the iTunes store, they show on the account's purchase history (Store > View Account on a computer's iTunes, or you can view the last 90 days purchases via http://reportaproblem.apple.com) ?
You could potentially log into the account on that computer's iTunes (Store > Sign Out to log out of your account, that will then change to a Store > Sign In option) and redownload them by clicking on the id towards the top right of the iTunes screen and Purchased on the popup :

Or do you have a different computer that you could redownload them on and then copy them over to that computer via a flash drive or memory card, and then add them to its iTunes via File > Add To Library ?

No, but you do not need iTunes to setup a new iPhone 5s. Create an iCloud backup for anything you want to transfer to the new iPhone.
