Kernal panics and graphics issues

Question

I have a 2008 MacBook Pro.  Over the last few weeks I've been getting strange graphics issues.  At first every other vertical line would go black and make the display unreadable.  That happened occasionally for a few days then would snap out of it and go back to normal.  The problem got worse, where the whole screen would shift colors, turning either a certain color like blue or green.  Then it started to cycle through the colors and would flicker like a bad MTv music video from the 1980s.  Eventually the display would always come back to normal, but I'm never sure for how long. 
Lately I've received a few kernal panics, which I think are related to this issue.  I've extracted the relevant logs below.  My hunch is that my graphics card is going bad, but I've never had that happen on a computer before.  Is this a known issue, is there anything I can do about it (replace / upgrade the card?)

The third-party system modification "Logitech Control Center" may be contributing to your problem. I suggest you back up all data, then uninstall it according to the developer's instructions. Reboot and see whether there's any improvement.
