Kernel Panic from USB after 10.5.6 and/or EFI 1.6 updates on MBP

For the last two-three months I have been developing code for a custom USB device with no problems whatsoever. After I upgraded to 10.5.6 and ran the EFI 1.6 update, my MBP throws a Kernel Panic like clockworks whenever I release the USB device.
This is how it happens:
I virtualize WXP through VMware fusion. Run Visual Studio 2005 and connect to my device (Cylinx EX=USB). The device runs perfectly well (sends/receives USB data) until the end of the program when handles to the device are released. When execution reaches the line that releases handles I get a kernel panic.
I had no problems before I installed the OS and firmware update.
I have no problems if I boot up WXP natively through BootCamp.
Anyone else seeing any USB kernel panics?

More info:
With still to many unknowns about the USB Kernel Panic while virtualizing(Fusion 2.0) Win XP (Visual Studio 2005) to drive a Cypress EZ-USB chip(Thesicon USB Driver) I did some testing.
Is it my code or the system?
I ran Thesicon's sample code for driving USB and the kernel panic occurred just as with my code. It happens exactly when the code releases the last handle to the USB device. Conclusion: My code is fine (it also runs perfectly on Bootcamp), it is the system.
Is it Fusion 2.0?
I went through the pain of installing Parallels v4 (Fusion is much nicer) and the problem occurred exactly as above. The USB device was recognized and sent and received data but a kernel panic happens as soon as the last handle to the device is released.
So my remaining question is whether it was the EFI update or OS X 10.5.6 update? Since everything runs well when I boot up using Bootcamp (which I assume is using the same firmware as when I boot up in OS X), and everything worked great before both updates, my number one suspect is the 10.5.6 upgrade.
Anyone else having USB problems while virtualizing, using Cypress USB chips or Thesicon Drivers.
For the moment I can boot XP through Bootcamp and survive, but my life is in OS X and its a pain to have to reboot back and forth.

  • Kernel panic on startup after Software Update

My mother's iBook G4 is unable to startup due to a kernel panic that began after the last software update.
On boot, the computer goes to the kernel panic (You need to restart your computer) screen after a few seconds of the Apple logo screen. It cannot boot in Safe Mode or Single User mode (it hangs after reporting system uptime.)
It can boot into Target Disk mode.
The iBook updated its software (Java for Mac OS X 10.5 Update 6, Security Update 2009-006, HP Printer Drivers Update 1.1.1 and Safari 4.0.4) yesterday, then restarted and kernel panicked.
This happened to my girlfriend's PowerBook right after an update too, and because she didn't have her system disks, I asked my Mom to send her copy of 10.5 to me from SF to Portland. Now, the same thing has happened while I was in SF, and the system disk is in Portland (meaning I don't have an OS disk.)
Is there some way to fix this? I have full access to the disk on the iBook through a FireWire cable and target disk mode on my MacBook Pro (but disk utility can't check permissions.) Seems that I should be able to fix it somehow. Ideas?

Install the combo update that you can download from the Apple download site at http://www.apple.com/downloads/ . Sometimes the software update function doesn't install the updates correctly, causing just odd behaviors. The combo update has all prior updates from that OS X version included. If possible, it's good to repair permissions before and after the combo update.

  • Kernel panics from software

    Dear users,
Since 4 weeks i got a new MacBook Pro which had kernel panics from the start.
After long disccusions i did get a new one (from the new release). But also this macbook has kernel panics.
So my conclussion is that some software is causing this problem but i don't know which.

That panic was not caused by third-party software. If the problem is recurrent, the possibilities are:
A stale or corrupt kernel cache
    A damaged OS X installation
A fault in a peripheral device, if any
    Corrupt non-volatile memory (NVRAM)
An internal hardware fault (including incompatible memory)
An obscure bug in OS X
You may already have ruled out some of these.
Rule out #1 by starting up in safe mode and then restarting as usual. Note: If FileVault is enabled, or if a firmware password is set, or if the startup volume is a Fusion Drive or a software RAID, you can’t do this. Post for further instructions.
You can rule out #2 and #3 by reinstalling the OS and testing with non-essential peripherals disconnected and aftermarket expansion cards removed, if applicable. Sometimes a clean reinstallation may solve a problem that isn't solved by reinstalling in place.
Corrupt NVRAM, which rarely causes panics, can be ruled out by resetting it.
If your model has user-replaceable memory, and you've upgraded the memory modules, reinstall the original memory and see whether there's any improvement. Be careful not to touch the gold contacts. Clean them with a mild solvent such as rubbing alcohol. Aftermarket memory must exactly match the technical specifications of the machine.
Apple Diagnostics or the Apple Hardware Test, though generally unreliable, will sometimes detect a fault. A negative test can't be depended on. Run the extended version of the test, if applicable.
In the category of obscure bugs, reports suggest that FileVault may trigger kernel traps under some unknown conditions. Most, though not all, of these reports seem to involve starting up from an aftermarket SSD. If those conditions apply to you, try deactivating FileVault.
Even if FileVault is not active, an aftermarket SSD may be the cause of kernel panics. Check the manufacturer's website for a firmware update.
Connecting more than one display is another reported trigger for OS X bugs.
If the system is not fully up to date, running Software Update might get you a bug fix.
In rare cases, a malformed network packet from a defective router or other network device can cause panics. Such packets could also be sent deliberately by a skillful attacker. This possibility is something to consider if you run a public server that might be the target of such an attack.
Otherwise, make a "Genius" appointment at an Apple Store, or go to another authorized service provider to have the machine tested. You may have to leave it there for several days. There isn't much point in doing this unless you can reproduce the panic, or if you can't, it happens often enough that it's likely to be repeated at the store. Otherwise you may be told that nothing is wrong.
Print the first page of the panic report and bring it with you.

There is something incompatible would be my guess. Did you disconnect all your peripherals in safe mode just to check?
You might now create a new temporary user account. Log into the new account. Do the panics still occur?
I would also visit The XLab FAQs and read the FAQ on diagnosing kernel panics. Sometimes they can be difficult to track down. Looking at each new panic log would help determine if it's cause is associated with the same extensions.

  • Repeated Kernel Panics on MBP17" after upgrading from SL (without anything else) to LION or MAVERICKS .

All  tentatives to upgrade from SL to LION or MAVERICKS on my MBP17" causes a KERNEL panic.
SL was reinstalled on several HDD and SSD WITHOUT anything else.
Any update to LiON or MAVERICKS will result in an immediate Kernel Panic.
    Any clue?
    Thanks

    Thanks for your quick answer.
I am the only administrator of my personnal MAC (only one account)
I can't even start in safe mode, there is no firevault nor firmware password , nor RAID volume.
Therefore I had to format the disk and reinstall SL that works like a charm. No way to access the console ….
Any attempt to install LION or MAVERICKS results in a KERNEL PANIC during installation ...
There is no apps installed, no peripheral attached,

  • Trying to do clean install from USB after kernel problem

    MacBook Pro 13" Mid 2010
    - was running OS X v 10.9
    - 2.4 GHz Intel Core 2 Duo processor
Machine was running extremely slow, unable to do anything with it. When booted to safe mode, lag issue was not present. Booted to Disk Utility, left column showed (photoshoped to show how it looked originally):
Verified and repaired Macintosh HD and disk1 - OS X Base System, no problems.
Booted Option R - Erased Macintosh HD,  tried to do new install from online, failed 3 times (took in excess of 8 hours each time). Still lagging, mouse movement very irratic.
Reset SMC, and NVRAM, some improvemet with mouse movement and speed
Read suggestions online to erase entire drive, boot from USB to DU, and install from bootable USB. Tried to do that. When in the boot from screen (Option) mouse lag not apparent. Chose USB, it opened up DU. Erased Macintosh HD again. Cannot delete or unmount OS X Base System, new screen looks like this:
From this screen, I exit back to the first screen and select Install OS X Mavericks, it gives me the choice to install on USB or Macintosh HD, I chose the latter, follow the steps, get a screen saying it's installing then it brings me back to DU after about 10 min. What am I missing here? Do I need to partition Macintosh HD creating a boot portion? I'm not a Mac person, I'm trying to fix this for my son.
Can I even do an install on Macintosh HD? I get that prior to booting to USB, I was using disk1, and so it couldn't be erased, but booting from the USB, would I not be using DU from the USB, and therefore able to clean the HD and start fresh? I'm guessing that the boot disk is causing the lag, and want to get rid of it.
Or is the disk showing disk3: OS X Install ESD accessible someone to do the install and I'm just going in circles?
I'd be extremely appreciative of any help you could provide, thank you.

Try this:  (1) On your Mac mount the disk in the Time Capsule (probably called "Data").  (2) In the "Data" volume locate and mount the "sparsebundle" image that contains the "old" backups.  (3) Click-and-hold on the Time Machine icon in the Dock, then choose from the pop-up menu the option to "Browse Other Time Machine Disks".  That should give you access to the old files.

  • Issue after kernel panic from update! Please help

Is anyone having issues with their computer after doing the reinstall? I too had the same problem yesterday as thousands of other people had. Took me 9 hours to reinstall lion. I was very happy when I realized I didn't lose all my data. (Guess I learned my lesson on backups) My problem is: Today after my computer is working again is is so stinking slow..... Im talking it has been 15 minutes to load the new mac commercial (30 second ad, on apple.com) In that 15 minutes it has only loaded 20 seconds of it. It is still trying to load the rest so I can stream it? I have never had an issue with speed. It is taking me nearly 4-5 minutes for every email I click just to pull up and read it. Something is wrong? I know it isn't my internet, we have 3 other computers in the house that work the way they always have. My laptop has always been the fastest of them all. I need help! Anyone have any suggestions? Is is possible if the update damaged the flash drive or something? All I know after 9 hours computer loss yesterday and my computer taking all day today to answer a few lousy emails is killing me, I need a solution. I have tried cleared the cache and reset safari with no luck.
    Please advise...

    HI Chris,
Kernel panics are sometimes causes by hardware issues. Probably a good idea at this point to check the boot disk for errors.
Open Disk Utility, in the Utilities folder in the Applications folder.
Select your disk, and click First Aid.
    Click Verify Disk.
If the disk needs repairs, do one of the following:
Insert your Mac OS X Install disc in your computer’s optical drive or an optical drive connected to your computer, and then restart your computer. When you hear the startup tone, hold down the C key until you see the progress indicator, which looks like a spinning gear.
Follow the onscreen instructions until the menu bar appears with the Utilities menu in it, choose Utilities > Disk Utility, click First Aid, and then click Repair Disk.
Checking a hard disk to see if it’s going to fail
You can check the health of your hard disk by checking its Self-Monitoring, Analysis, and Reporting Technology (S.M.A.R.T.) status in Disk Utility. Most disk drives include S.M.A.R.T. hardware. Developed by IBM, S.M.A.R.T. hardware allows hard disks to test themselves for potential problems.
Starting up in “safe mode” may help you to diagnose problems you’re having with your computer.
To start up in safe mode:
Shut down your computer and wait 10 seconds.
    Press the power button.
Immediately after you hear the startup tone, hold down the Shift key.
You should press the Shift key as soon as possible after your hear the startup tone, but not before.
Release the Shift key when you see the gray Apple logo and progress indicator (spinning gear).
To leave safe mode, restart the computer normally without holding down any keys during startup.

    If the boot disk appears to be ok, you could try installing the 10.5.6 combo update available here;

  • 1-2 Kernel panics a day after 10.5.7 update

After updating to 10.5.7 I'm getting 1-2 kernel panics per day. I have tried the following:
    * Downloaded 10.5.7 combo and reinstalled
    * Run the memory test by booting up on the CD that came with the machine
    * Run disk utility and verirified the hard disk is ok
    * Repaired all permissions
    * Reset the PRAM by pressing Command, Option, P, and R during boot up
None of the above has worked.
The kernel panic occurs during various processes but the log always says the following:
Kernel loadable modules in backtrace (with dependencies):
    com.apple.driver.AppleIntelCPUPowerManagement(76.0.0)@0xb11000->0xb2ffff
Below is one example of the panic log.
Any suggestions on how I can fix this?
    Thanks!

I'm guessing something is corrupted assuming it's not a hardware problem. I would try this:
How to Perform an Archive and Install
An Archive and Install will NOT erase your hard drive, but you must have sufficient free space for a second OS X installation which could be from 3-9 GBs depending upon the version of OS X and selected installation options. The free space requirement is over and above normal free space requirements which should be at least 6-10 GBs. Read all the linked references carefully before proceeding.
    1. Be sure to use Disk Utility first to repair the disk before performing the Archive and Install.
Repairing the Hard Drive and Permissions
Boot from your OS X Installer disc. After the installer loads select your language and click on the Continue button. When the menu bar appears select Disk Utility from the Installer menu (Utilities menu for Tiger.) After DU loads select your hard drive entry (mfgr.'s ID and drive size) from the the left side list. In the DU status area you will see an entry for the S.M.A.R.T. status of the hard drive. If it does not say "Verified" then the hard drive is failing or failed. (SMART status is not reported on external Firewire or USB drives.) If the drive is "Verified" then select your OS X volume from the list on the left (sub-entry below the drive entry,) click on the First Aid tab, then click on the Repair Disk button. If DU reports any errors that have been fixed, then re-run Repair Disk until no errors are reported. If no errors are reported, then quit DU and return to the installer.
    2. Do not proceed with an Archive and Install if DU reports errors it cannot fix. In that case use Disk Warrior and/or TechTool Pro to repair the hard drive. If neither can repair the drive, then you will have to erase the drive and reinstall from scratch.
    3. Boot from your OS X Installer disc. After the installer loads select your language and click on the Continue button. When you reach the screen to select a destination drive click once on the destination drive then click on the Option button. Select the Archive and Install option. You have an option to preserve users and network preferences. Only select this option if you are sure you have no corrupted files in your user accounts. Otherwise leave this option unchecked. Click on the OK button and continue with the OS X Installation.
    4. Upon completion of the Archive and Install you will have a Previous System Folder in the root directory. You should retain the PSF until you are sure you do not need to manually transfer any items from the PSF to your newly installed system.
    5. After moving any items you want to keep from the PSF you should delete it. You can back it up if you prefer, but you must delete it from the hard drive.
    6. You can now download a Combo Updater directly from Apple's download site to update your new system to the desired version as well as install any security or other updates. You can also do this using Software Update.

The kexts don't seem to referenciate any third party software. It looks like I've problems with the GPU and/or iPhoto, but I don't understand this kind of things properly. I must point here that, before the update, I don't have any problemas at all. Also, I've seen in the support forums that there are bugs on Nvidia graphic cards in Mountain Lion. So... my questions are: a) Is this some kind of bug? b) Can I do something to solve this issue (I've already checked Activity Monitor, reset PRAM, repaired permissions, erased Macintosh HD and restored my hard drive... without results)? c) Could you help me?
    Thanks!

That panic was not caused by third-party software. If the problem is recurrent, the possibilities are:
A stale or corrupt kernel cache
    A damaged OS X installation
A fault in a peripheral device, if any
    Corrupt non-volatile memory (NVRAM)
An internal hardware fault (including incompatible memory)
An obscure bug in OS X
You may already have ruled out some of these.
Rule out #1 by booting in safe mode and then rebooting as usual. Note: If FileVault is enabled, or if a firmware password is set, or if the boot volume is a software RAID, you can’t do this. Post for further instructions.
You can rule out #2 and #3 by reinstalling the OS and testing with non-essential peripherals disconnected and aftermarket expansion cards removed, if applicable. Sometimes a clean reinstallation (after erasing the startup volume) may solve a problem that isn't solved by reinstalling in place, without erasing.
Aftermarket memory must exactly match the technical specifications for your model. Memory that is either slower or faster than specified may be incompatible.
The Apple Hardware Test, though generally unreliable, will sometimes detect a fault. A negative test can't be depended on. Run the extended version of the test.
In the category of obscure bugs, reports suggest that FileVault may trigger kernel traps under some unknown conditions. Most, though not all, of these reports seem to involve booting from an aftermarket SSD. If those conditions apply to you, try deactivating FileVault.
Connecting more than one display is another reported trigger for OS X bugs.
In rare cases, a malformed network packet from a defective router or other network device can cause panics. Such packets could also be sent deliberately by a skillful attacker. This possibility is something to consider if you run a public server that might be the target of such an attack.
If none of the above applies, make a "Genius" appointment at an Apple Store to have the machine tested. You may have to leave it there for several days. There isn't much point in doing this unless you can reproduce the panic, or if you can't, it happens often enough that it's likely to be repeated at the store. Otherwise you may be told that nothing is wrong.
Print the first page of the panic report and bring it with you.
Back up all data on the internal drive(s) before you hand over your computer to anyone. If privacy is a concern, erase the data partition(s) with the option to write zeros* (do this only if you know how to restore, and you have at least  two independent backups.) Don’t erase the recovery partition, if present.
Keeping your confidential data secure during hardware repair
*An SSD doesn't need to be zeroed.

  • Kernel Panic From Sleep Mode

The third-party system modification "SX-Virtual Link" may be contributing to your problem. I suggest you back up all data, then uninstall it according to the developer's instructions. Reboot and see whether there's any improvement.
Back up all data before making any changes.

  • Kernel panic from com.apple.filesystems.afpfs

Looking back at my panic logs over the last two months, I have 15 kernel panics, 11 of which appear to be coming from com.apple.filesystems.afpfs. This appears to always happen when I mount a drive over AFP, am copying files, and the network connection gets disconnected somehow (system goes to sleep, disable wifi).
Help? I figure I can reduce the amount that this happens manually but a dropped AFP connection shouldn't be cause for a kernel panic.

    I would suggest trying this.
First, repair your hard drive and permissions:
Boot from your OS X Installer disc. After the installer loads select your language and click on the Continue button. When the menu bar appears select Disk Utility from the Installer menu (Utilities menu for Tiger and Leopard.) After DU loads select your hard drive entry (mfgr.'s ID and drive size) from the the left side list. If no errors are reported click on the Repair Permissions button. Wait until the operation completes, then quit DU and return to the installer. Now restart normally.
If DU reports errors it cannot fix, then you will need Disk Warrior (4.0 for Tiger, and 4.1 for Leopard) and/or TechTool Pro (4.6.1 for Leopard) to repair the drive. If you don't have either of them or if neither of them can fix the drive, then you will need to reformat the drive and reinstall OS X.
    Next, clear system caches;
Boot into single-user mode. After startup is completed you will be in command line mode and should see a prompt with a cursor positioned after it. At the prompt enter the following then press RETURN after each commandline:
    /sbin/fsck -fy
If you receive a message that says "*** FILE SYSTEM WAS MODIFIED ***" then re-run the command until you receive a message that says "** The volume (nameofvolume) appears to be OK." If you re-run the command more than seven times and do not get the OK message, then the drive cannot be repaired this way.
If you were successful then enter:
    /sbin/mount -uw /
    rm -rf /System/Library/Caches/*
    rm -rf /System/Library/Extensions.mkext
    rm -rf /Library/Caches/*
    reboot
I suggest you print out the above to be sure you do not make any errors when entering the commands. When in single-uwer mode you have 'root' access, so it's important that you not make any mistakes.
See if the above helps. I would also suggest using Ethernet instead of wireless, if you can, to see if the problem is limited to the wireless side of the network.
Also visit The XLab FAQs and read the FAQ on diagnosing kernel panics.

  • Kernel Panic on shutdown after OS Lion install - why?

    Hi,
I just installed Lion on my MBP and restored programs/data from time machine.
After the 2nd or so start up, I get a kernel panic everytime now upon shutting down my MacBook. Message included below.
Other users on the same machine work okay so far.
Any ideas from the report below? I cannot make sense of it...
    Thanks!

Presuming that those references to VMware versions 3.1.2 match the version of VMware Fusion that you have, I'd try updating it to version 3.1.3.  I'd also look for an update for your Logitech driver or uninstall it if you're no longer using a Logitech device.

  • Kernel Panic from system file permission changes

A friend and i were trying to share some image files with each-other so we firewire connected our two macs... (both mac book pros mine a intel core on snow-leopard hers the latest is7 operating system -prob Lion) when the corresponding computer did not appear on our desktops I entered "sharing" console on my system prefs and attempted to allow her computer to see and read/write mine... I got a pop up prompt asking if i wished to share all my internal files and I said "yes"  Shortly thereafter a repeat prompt kept popping up on my computer saying something like ~"can't share system files" I pressed "ok" each time it appeared and after a few pop ups it all seemed ok...
By now as a backup attempt to share my files i had plugged in my external (open permissions) hard-drive which at this point suddenly told me i am not allowed to read or write on it... I ejected it and since the whole system was still spinning and wanting to share permissions I force-quit the finder and selected "shut down" my system to start fresh but when attempting to restart finder I first a few moments later I got that - multilingual kernel panic prompt
it won't start up - it won't boot from disc (at start i hold down option and insert operating disc) it wont start up (I've depressed shift after start and arrived at a spinning wheel that never ends... i'm also out of my warranty and hoping to solve this without paying for a new mac

Unfortunately, the log files are full of scary-sounding messages. Unless you have some specific problem other than the logs, there is usually no need to worry about it. Sometimes, a problem may be reflected in the logs, but you can't start with the logs. You'll never finish.

  • Kernel Panic - nothing changed after volume repaired

    Hi,
I have Kernel Panic and can only boot up under Safe Mode.
So, a bit of research via this forum, and I inserted the CDs belonging to my nephew to boot up using Mac OS disk as start-up disk.
After serveral tries with "C" key held down, finally got into the page with a nice background, and went on to Disk Utility.
So far so good, proceed to Repair, and the volume repaired.
Re-started, but back to kernel panic screen again.
    Someone please help me.
I don't remember having my own CDs that come with the purchase.
My battery is dead so I am using direct AC power permanantly.
I thought problem solved if the volume is repaired.
I can't install using my nephew's installation disk either.
Someone please tell me what to do next. Thx.

Re: "kernal panic" or "gray screen of death"
I've just started having this problem on 2 of my 3 Macs. They are an eMac and an iBook, about 4 or 5 years old. Each is running OS 10.4 My 2 year old iMac, running 10.5, is fine (so far).
I get the gray screen with the instructions to restart (in four languages).
Since it just started happening on two separate computers, I would think that a hardware problem is ruled out (unless it's an extraordinary coincidence). Also, I see a lot of discussion about people having this problem after upgrading to Snow Leopard. Obviously, that's not the case either with these two older computers.
I've just installed a new Airport Extreme and an Apple TV in my house. When I did it, I created a new network rather than reconfiguring the old one. Could that be the cause???
For now, I've zapped the PRAM on each machine and that seems to have helped.

  • Constant kernel panics with USB

I have become increasingly frustrated and angry at my MacBook in the last few weeks. Nearly every time I plug in a USB hub, I get a kernel panic. When I send the report to apple, this is what comes up near the bottom of the window:
    Backtrace continues...
Kernel loadable modules in backtrace (with dependencies):
    com.baltaks.driver.DoubleCommand(1.6.5)@0x3441b000->0x34420fff
    dependency: com.apple.iokit.IOHIDFamily(1.5.3)@0x709000
    com.apple.iokit.IOHIDFamily(1.5.3)@0x709000->0x741fff
Sometimes there's a USBHUB written in there. What is going on? I've tried switching from a rather generic hub to a Belkin one, and most recently I just plugged in my aluminum keyboard and immediately got a KP. They've been happening with regularity over the past while, all when I unplug or plugin my USB hub with an iPod touch, keyboard/mouse, USB drive and headset (though recently I've been removing peripherals).
I'm about to do an archive and install (assuming my MB works long enough for me to download the combo updater) but want to kill this bug soon.

I'm afraid not. Kernel panics are notoriously difficult to diagnose.
Have you changed your system in anyway since the panics began (a new peripheral/hub, installed new RAM etc)?
This site has a list of general ideas to help try and figure out what's going on:
    http://www.thexlab.com/faqs/kernelpanics.html
Maybe something in there will work for you. Good luck!
