Livecycle Designer Error- everytime I open FORMS-HELP!

Can someone help me please?!
I can't seem to find anyone that can.
I have CS3 and everytime I open forms this happens. I have uninstalled and reinstalled it 4 different times-still continues...
I get this ERROR:
Adobe LiveCycle Designer has encountered a problem and needs to close. We are sorry for the inconvenience.
Error signature:
AppName: formdesigner.exe AppVer: 8.0.2073.1 ModName: unknown
ModVer: 0.0.0.0 Offset: 04d58113
then I get a second one saying:

Can anyone who is having these issues send the crash dump to [email protected] and I will forward them to R&D for investigation.

Sometimes it is useful to capture an ad-hoc minidump file from FormDesigner when it crashes. Here are instructions for doing this on Vista, and XP.
On Vista 32 bit, wait for the app to crash, and while the "your app has crashed" dialog is still visible, go to Task Manager process list, right click on FormDesigner .exe, and select "Create Dump File' from the context menu. After the dump file is written, its location will be shown in a second dialog box. The "designer crashed" dialog can then be dismissed.
For Vista 64 bit, I think the dump files produced this way are probably not useful, because they will show stacks for the "64 bit side" of the 32 bit process, which is not what we want to look at. For Vista64, it is probably necessary to attach a real debugger to FormDesigner .exe and write a dump file.
On XP, if the default drwtsn32 post-mortem debugger is installed, the dump of the last crash is available at "C:\Documents and Settings\All Users\Application Data\Microsoft\Dr Watson\user.dmp". If the user has installed a different post-mortem debugger (e.g. visual studio) or disabled things somehow, then an ad-hoc dump can be made by waiting for the "designer crashed" dialog, and leaving that open. Then go to task manager's process list and note the process ID of FormDesigner .exe. Then bring up a command prompt and type:
Drwtsn32 -p designer_PID
where designer_PID is the process ID of FormDesigner .exe found in task manager. After that, the "designer crashed" dialog can be closed, and the dump will be found in "C:\Documents and Settings\All Users\Application Data\Microsoft\Dr Watson\user.dmp".
Along with the crash dump file, you should provide the exact build number of the FormDesigner executable you are running (e.g. 8.2 is not sufficient.) You can find the build number by right clicking on FormDesigner .exe and looking at the "Details" tab. It's the "Product Version" entry.

LiveCycle Designer 8/ Acrobat Pro 8-form data

Not skilled in using the LiveCycle Designer included in Acrobat Pro 8, I have been struggling for a couple of weeks, trying to format a sample form and export data to a .csv file to read in Excel or notepad, once saved in .pdf and completed. I have the basics, but can't accomplish the more advanced. So far, I have used LiveCycle Deisgner to create a one page table form, with a header, 20 columns and 16 rows. The header cells are all text boxes labeling the columns below them. The rows have mostly text input cells/boxes, a few drop- down list boxes.
I can complete, in .pdf, a couple of sample lines with made-up names, other data as a test. It will save, the using manage form data>merge data files into a spreadsheet, I can create a .csv and view it in Excel.
Here's where the difficulty begins. I need the .csv to display every cell/box, even if blank, and in the correct row. The form data will be used by a genetics program's import module to import data and needs to be either a .txt or .csv tab, comma or other symbol (for example, @) delimited file, where blanks are represented by one extra delimiter (as a spacer) for each blank. The .csv data display is anything but that. The header runs across the first .csv row with names like :"form1[0].ID1[0]" while the only other row showing is all of the sample data from the .pdf's 2 completed sample data lines, all in one row, with some blank cells/boxes shown, and some not shown.
Since eventually, once the form is "up and running," it can collect 50+ rows, I was hoping not to have to manually edit the .csv to get it into the acceptable format for the other program's import module.
I am at a loss about how to set up the form in LiveCycle Designer so that the header row doesn't export to the .csv (I am unable to find the data binding controls for these text/header boxes to set them to "none."), the blank cells/boxes all register, rather than only some, and the row 1 data goes on the .csv's row 1 and the row 2 data to the .csv's row 2, etc., rather than an endlessly long single row.
I've already searched forum's, plus the help files for LiveCycle Designer and Acrobat, with no success.
Any ideas, solutions or places where I can check further would be greatly appreciated.

Hi,
With LC Designer you can develop a form which has data connections to databases and/or web services.
The issues to consider are: what version of Acrobat will the user have; and if the user only has the Reader then how are you Reader Extending the form.
User has the fulll version of Acrobat:
user can open form;
fill it in;
print it;
data connections to databases and web services will work;
user can save the form.
User has the Reader (form not Reader Enabled)
user can open form;
fill it in;
print it;
data connections to databases and web services will NOT work;
user CANNOT save the form.
User has the Reader (form Reader Enabled using the full version of Acrobat before deployment. See licence restrictions)
user can open form;
fill it in;
print it;
data connections to databases and web services will NOT work;
user can save the form.
User has the Reader (form Reader Enabled using the full LC Enterprise Suite)
user can open form;
fill it in;
print it;
data connections to databases and web services will work;
user can save the form.

LiveCycle Designer 8.0 Drop-down menu help - newbie

I am new with LiveCycle and do not know much about coding and the like. I've search the forums and have not found quite what I need. (It might be because I don't know the correct terms to search - so I apologize in advance if there was a previous posting regarding this)
I am creating a form for lab tests for referring physicians to fill out.
for simplicity, I will just use the state, city example.

I have dragged/dropped a drop-down menu icon onto the form, gone through the Palette>Object>Field>List Items to compile the list of states (in my real case, a list of lab tests)
How can I get the second drop down menu to show the corresponding cities based on the "state" selected in the first drop down menu? So that when I choose "California" I have the option in the second drop-down menu to choose from "Los Angeles," "Orange County," "Long Beach," etc
VERSUS
if I select "Texas" the list will retrieve "Houston," "Sugarland," "Dallas," etc.
I read about hardcoding and although I'm not quite sure what that means, I think that is what I will be doing as I will not be referencing outside of my form.
Also, the items in these menus are long/descriptive in nature, is there a way to refer to the lists in the code without having to type everything out? In my form (instead of "state") I will have lab tests "Test_1" with CPT codes attached, that is the list includes items like:
93880 - carotid duplex scan, bilateral
93882 - carotid duplex scan, unilateral
and in drop-down list 2 I have items like:
454.0 - varicose vein with ulcer
454.1 - varicose veins with inflammation
it would be such a pain to type each out in the code, but if there are no other ways around it, that is fine too.

To prevent an "accidental" changes, I would like the form to be a "live form" for people to fill out, but upon return, the form to be a static PDF file, that is, the person receiving the email does not have the option to make any changes (purposeful or accidental), is there a way to do this?

The form I am creating is a Vascular Lab request form, where the test ordered (drop down box 1)has a CPT code to reference the test being ordered (i.e., 93880 - Carotid Duplex Scan, bilateral) and the test has a set of diagnoses (ICD-9 codes) in drop down box 2, associated with that test (i.e., 459.81 - Venous Insufficiency)
I have found that I do not have a problem with the "diagnoses" but I cannot add the descriptive test select in the first place. That is, the code works fine if I leave it as "93880" but not "93880 - description." I have also run into the problem that I cannot use dashes or underscores, nor can I type more than 17 characters otherwise the code no longer works.
Is there something I need to add or remove in order to make this work?

Can i use Response method in livecycle designer to render or open pdf doc?

Can anybody please tell me is it possible to use response method available in the adobe livecycle designer which takes the input value to open the pdf doc.
Case 1: if we send adobe form throgh mail as pdf doc,then at the receiver side if the user open the pdf doc a popup should come asking for the input value.This popup will be from the response method.
Case 2:if we save the adobe form as pdf doc on to our desktop and then if we try to open the pdf doc a popup should come asking for input value.
Is it possible to do ?
If yes then in which event of livecycle designer we should write that response method and in what way?
If no,is there any other way to do this?

Yes i have tried in other events of livecycle designer.
In Formready event -> if i do preview pdf,first it will display form and then it ll ask for password.But with this event only if i save the pdf to my local desktop and try to open it ll not ask for password anywhere.
In Layoutready event ->Pdf preview in designer is working fine,but when i save it my local desktop and try to open it ll not ask for passward.
Could anybody tell me how to do it.

     When i try to open de layout of the Adobe LiveCycle Designer with the SFP transaction i get this Windows Error Message:
    Adove LiveCycle Designer: FORMDE~1.EXE Error de Aplicación
    La instrucción en "0x7830e113" hace referencia a la memoria en "0x0000048c". La memoria no se puede "read".
This has been working fine... but 2 daysa go... im getting this message error and can´t load the LayOut View.
I was trying in other machines. and this work.. fine..

Adobe LiveCycle Designer error in WebDynpro

In the Adobe interactive form I am trying to add some elements from the library. As soon i drag and drop some element in the form I get message as below.

Error occures when opening form in planning

This occurs only for one of my applications, other work fine.

Have you migrated to 11.1.2.1 lately?
Restart EAS and Planning. Login to Planning as a Planning admin, then login to EAS with the Planning/Essbase admin - Do not use the default admin user.
Check if you are able to associate the outline thereafter.

We were using Adobe Livecycle Designer 7.0 for creating forms, what is new product replacement?

We were using Adobe Livecycle designer 7.0 (circa 2004) for creating our own dynamic forms. The program has since failed to work, we cant locate the old disk. What is the new product replacement that will allow us to build new forms from scratch with our own fields and data collection?

Annoying error everytime i open Safari

Hi there everyone. Really hope someone can help me out here. I have looked everywhere to see if someone else has had this error and I can't seem to find anything. I don't know what is causing it but it has been awhile. Most likely when I moved to SNow Leopard long time ago but haven't delt with it cause I have been gone for awhile. Anyhow, I open safari and this error comes up everytime:
Safari 4.0.5 (v6531.22.7) has not been tested with the plugin PCB (null)(v19). As a precaution, it has not been loaded. Please contact plugin developer for further information.
So thats it. The problem is I don't even know what plugin its talking about cause it doesn't ring a bell or come up on my plugin list. So hopefully some genius is out there to help me. Really appreciate any advice someone has. Thanks

~ means your Home Folder so the path is; MacintoshHD/Users/YourName/Library/Internet plugins.
If it's not there, try deleting the Safari preference file and see if that makes a difference.
Go to the Safari Menu Bar, click Safari/Preferences. Make note of all the preferences under each tab. Quit Safari. Now go to ~/Library/Preferences and move this file com.apple.safari.plist to the Desktop. Relaunch Safari. If it's a successful launch, then that .plist file needs to be moved to the Trash.

Problem opening form -Help!

This a uk govenment tax form that cannot open, although I managed to open it and save data (save as) three weeks or so ago. But now its no longer working.
I've downloaded a new form and tried to open no joy, downloaded and saved on another computer (both win 7) no joy, uninstalled and reinstalled adobe reader X no joy, uninstalled and installed adobe 9.4 still no joy.
I get the message:
"Adobe Reader has stopped working.
A problem caused the program to stop working correctly.
Windows will close the program and notify you if a solution is available."

The gov helpline insists their form is fine and that its a win 7 problem.
Help! Its a catch 22 I have to file or get fined but I can't open the pdf to file.

Your Adobe Reader program is corrupt as stated in AcroRd32.exe. Please try to download the reader and install. Please de-install your Adobe Reader first.
Government cannot force you to own a COMPUTER. Form has to be available somewhere else.
Go to another computer, work, friends, public library and download the form from there. THEN deal with your computer issues. Or pay fine and still need form......

I'm getting an error everytime I open iTunes

Recently, I had a problems with my DVD drives. A solution I came across was to delete some registry keys that related to them. After fixing that issue, I got an iTunes message that said this:
Warning!
The registry settings used by the iTunes drivers for importing and burning CDs and DVDs are missing. This can happen as a result of installing other CD burning software. Please reinstall iTunes
I found that the fix for this was to install GEAR drivers (which I did) and I also reinstalled iTunes twice since then. I still get this error every time I start iTunes. However, I can, in fact, import and burn CDs and DVDs with iTunes even though the registry keys have not reappeared. This is really annoying because it makes iTunes take more than twice as long to load.
So if someone could help me either get rid of the error message or restore the registry keys, either would solve my problem. I realize that it's hard to help with the registry keys without knowing exactly what they are and I'm sorry I didn't write them down. All I remember is that they had something to do with an upper and a lower something or other.

Ipod won't start and not able to restore ( it shows network error everytime i try restoring) HELP !!!!!

I tried restoring my ipod ....
Now every time i try to switch it on .... the apple logo appears .....stays for a long time and then the device turns off
Every time i try to download a restore , it says network error ..
I have tried re-installing my itunes (it is the latest version) , disabled my firewall .
PLEAse HElp !!!!!!

LiveCycle Designer 7 : Saving the PDF form for the users ?

Hi,
When building a form, which will be filled by users, is there any way that the users can be offered the possibility to save their data within the PDF so that they can send the complete form by emails or through a other files exchange media (FTP, WEB, Flashdisk... )
Although having the possibility to sent only the XML data via email (which is great and we would like to keep this option) we fear that some of our users, which are filling the form, might not feel confident with this and might want to sent their completed form too ?

I was able to make this work for the first time yesterday. Assuming that you have Abobe Acrobat 8.0, once you have built the form...using adobe 8 go to the Advanced menu and select "Enable Usage Rights in Adobe Reader". The first time that I did this, and then attached the form to a user of reader - they were not able to save the completed form. So - I went to 'Forms' on the menu and selected "distribute form". Then the user of adobe was able to saved that form and changes that they made.
The distribute function wants you to email from there as a default. I had trouble because our default mail client is Novell. Still can't get that to work right. BUT - you can distribute the form and save it to your drive for distribution later. I then attached it to an email and sent it out to users.
Be sure to walk your end user through - using save as and saving it to their computer. My folks are used to NOT being able to do that so they still said they couldn't save the completed form.
Sorry if this explanation is very basic - but I am not at all a programmer! :) This is what worked for me. I spent hours on this site yesterday trying to work out the email piece and stumbled into this solution.
