FORCE IPAD INTO RECOVERY MODE
1. Turn off iPad
2. Turn on computer and launch iTunes (make sure you have the latest version of iTune)
3. Plug USB cable into computer's USB port
4. Hold Home button down and plug the other end of cable into docking port.
DO NOT RELEASE BUTTON until you see picture of iTunes and plug
5. Release Home button.
ON COMPUTER
6. iTunes has detected iPad in recovery mode. You must restore this iPad before it can be used with iTunes.
7. Select "Restore iPad"...
Note:
1. Data will be lost if you do not have backup
2. You must follow step 1 to step 4 VERY CLOSELY.

    Iam locked out of my Ipad. It wants a passcode that I do not have. I have tried the re-boot and that did not work. This is a hand me down ipad and it was working

    The only "official" way to recover from a lost or forgotten passcode to restore the iPad as a new device. Doing so completely erases the iPad.
    However, if you are really running iOS 3.1.3 as your system information suggests, you may be able to extract the passcode with techniques you can find on You Tube etc.
    Later versions of iOS encrypt the restrictions passcode very securely so such techniques will no longer work.

    My husband changed the Apple ID on our computer.  For some reason, iCloud has the old Apple ID and it won't let me change it.  I've been locked out of my iPad twice and had to restore it because of iCloud.  How do I change the Apple ID (not the password)?

    In order to change your Apple ID or password for your iCloud account on your iOS device, you need to delete the account from your iOS device first, then add it back using your updated details. (Settings > iCloud, scroll down and hit "Delete Account")
    Providing you are simply updating your existing details and not changing to another account, when you delete your account, all the data that is synced with iCloud will also be deleted from the device (but not from iCloud), but will be synced back to your device when you login again.
    In order to change your Apple ID or password for your iCloud account on your computer, you need to sign out of the account from your computer first, then sign back in using your updated details. (System Preferences > iCloud, click the sign out button)
    In order to change your Apple ID or password for your iTunes account on your iOS device, you need to sign out from your iOS device first, then sign back in using your updated details. (Settings > iTunes & App store, scroll down and tap your ID)
    If you are using iMessages or FaceTime, you will also need to log out and into your ID there too.

    I got locked out from my iPad mini during my initial setup.  Keep ask me for passcode but I haven't setup one.  Reboot and restart didn't solve the problem, tried connect to iTunes but it won't let me because it requires the passcode.  Tried my Exchanged password, Apple ID password and my iPhone passcode but none work.  Any idea to resolve this?

    Try and force your iPad into Recovery Mode
    1. Disconnect the USB cable from the device, but leave the other end of the cable connected to your computer's USB port.
    2. Turn off the device: Press and hold the Sleep/Wake button for a few seconds until the red slider appears, then slide the slider. Wait for the device to turn off.
    3.While pressing and holding the Home button, reconnect the USB cable to the device. The device should turn on.
    4. Continue holding the Home button until you see the "Connect to iTunes" screen. When this screen appears, release the Home button. iTunes should alert you that it has detected a device in recovery mode. Click OK, and then click Restore to restore the device.
    Note: Data will be lost.

    Here's the Apple support page on this. If the Restpre process doesn't work you need to put your device into Recovery mode. Read through all the steps and follow them carefully.

    Saw this solution on another post about an iPad in a school enviroment. Might work on your iPad so you won't lose everything.

    ‘iPad is disabled’ fix without resetting using iTunes
    Today I met my match with an iPad that had a passcode entered too many times, resulting in it displaying the message ‘iPad is disabled – Connect to iTunes’. This was a student iPad and since they use Notability for most of their work there was a chance that her files were not all backed up to the cloud. I really wanted to just re-activate the iPad instead of totally resetting it back to our default image.
    I reached out to my PLN on Twitter and had some help from a few people through retweets and a couple of clarification tweets. I love that so many are willing to help out so quickly. Through this I also learned that I look like Lt. Riker from Star Trek (thanks @FillineMachine).
    Through some trial and error (and a little sheer luck), I was able to reactivate the iPad without loosing any data. Note, this will only work on the computer it last synced with. Here’s how:
    1. Configurator is useless in reactivating a locked iPad. You will only be able to completely reformat the iPad using Configurator. If that’s ok with you, go for it – otherwise don’t waste your time trying to figure it out.
    2. Open iTunes with the iPad disconnected.
    3. Connect the iPad to the computer and wait for it to show up in the devices section in iTunes.
    4. Click on the iPad name when it appears and you will be given the option to restore a backup or setup as a new iPad (since it is locked).
    5. Click ‘Setup as new iPad’ and then click restore.
    6. The iPad will start backing up before it does the full restore and sync. CANCEL THE BACKUP IMMEDIATELY. You do this by clicking the small x in the status window in iTunes.
    7. When the backup cancels, it immediately starts syncing – cancel this as well using the same small x in the iTunes status window.
    8. The first stage in the restore process unlocks the iPad, you are basically just cancelling out the restore process as soon as it reactivates the iPad.
    If done correctly, you will experience no data loss and the result will be a reactivated iPad. I have now tried this with about 5 iPads that were locked identically by students and each time it worked like a charm.

    You must restore the device with iTunes on a computer. You want to use the same computer that you always sync with so that you can restore your app data and settings. You can restore with any other computer, but you will lose everything on the iPad.
