Microsoft Management Console Crashes

the MMC on my Windows Server 2012 has been extremely unstable, I keep getting this error: 
Any suggestions - I am reviewing the related threads now.

Don't know how I ended up with two account names...
I ran a few of the windows command line tools that check system files and they seemed to come back without any issues:

I am having the same issue on my other AD server - started right around the same time. . . Both machines are VMs...
